Summary
JOB PURPOSE To be a source of highly specialist psychosocial expertise; providing specialist guidance and support to psychosocial colleagues and the multidisciplinary palliative care team and managing a small caseload of complex palliative care cases. To provide senior leadership and operational management to the multidisciplinary Family Support Team, ensuring the highest quality social work, spiritual support, counselling and bereavement provision that is safe, caring, responsive, effective and well led. To lead on user involvement and engagement to ensure hospice services are truly person-centred and tailored to the needs of our local community as well as individual recipients of our support and care. To work in partnership with the Director of Clinical Services to actively identify and create opportunities to develop Family Support Team in the pursuit of widening access and optimising quality of life for palliative patients and their loved ones, in line with the Hospice vision and strategic plan.
