Summary


The opportunity has arisen for us to recruit a dedicated professional who would like to be given the opportunity to make a difference to their patients. The post holder will continue to develop the Bariatric CNS Nursing service, ensuring this encompasses the needs of the patients and is in line with Standards of good practice.

This post will support the Lead CNS with the development and implementation of specialist programmes of nursing care for individual patients and the client group as a whole.

Be proactive in the management of patients, operating a call and recall system and provide a level of advanced and comprehensive nursing assessment based on specialist knowledge and experience.
• Provides a specialist and holistic assessment of patients and carers needs following referral. Provides specialist care and advice for patients and carers and assists the multidisciplinary team in delivering and evaluating the interventions/ plan of care, reviewing and amending this plan as appropriate.
• Act as the patient’s advocate maintaining privacy and dignity.
• Provide advice and support to ward staff and learners in all aspects of the delivery of high-quality care.
• Attend Bariatric clinics, providing support, advice and education to patients/ significant others in the management of their care
• Contribute to operational and strategic developments in the Bariatric service. To provide Nurse Specialist service as part of the Multi-Disciplinary Bariatric Service
• Instigate actions to the development of a relationship between the patient/ carer and members of the MDT, in order to enhance communication
• Assists in the development and implementation of guidelines and pathways. Supporting the delivery of care, in line with national recommendations.
