We are an exceptional multidisciplinary team working with both the health and social care professionals. An exciting opportunity has arisen for a senior Practitioner to join and lead a team of dedicated social workers, who can provide care and support to older People's with Mental health needs.
Key skills include:
- Writing care and support plans, supervising staff, and managing complex casework for older people with mental illness and dementia. Excellent written and verbal communication is also required.
- The successful applicant will have two years post qualification experience within adult social care. Please see JD and

- You are required to have a social work qualification and to be registered with Social Work England.
- Disclosure and Barring Service (DBS) check is required, and at an Enhanced level
