Summary


An exciting opportunity to be part of UHCW's Integrated Musculoskeletal Service has become available. This role will give the successful candidate the chance to work across the MSK pathway, including the MSK assessment and triage, First Contact Practitioner (FCP), and Physiotherapy Outpatient Services.

The successful candidate will also have the opportunity to take a lead role within their own specialist field/area of clinical interest. This may include developing expertise and advanced practice skills, for example within upper limb, lower limb, spinal or persistent MSK pain

This Role is listed as a Band 8a role, but for the right candidate we would consider the opportunity to work as a Trainee Band 7-8a, where candidates would be supported to develop their skills towards Advanced Practice by demonstrating this through a portfolio which follows the capabilities and competencies detailed for Advanced Practice. Please note that the progression period if appointed as a Trainee band 7 would be for a maximum of 4 years before being uplifted to band 8a.

Be responsible for the specialist assessment, triage and management of patients within the MSK Single Point of Assessment, General Practice and in a sub-specialty area of MSK practice, either electronically, by telephone or face-to-face. Manage own or shared complex caseload and work independently and autonomously taking responsibility for all aspects of your intervention.

Undertake advanced therapeutic assessment of patients presenting with complex physical and psychological conditions, using advanced clinical reasoning skills to provide a diagnosis, develop and agree a goal orientated management plan and deliver individual and group treatment programmes.

Ensure clinical performance review takes the form of regular training, support, clinical reasoning and peer review.

Promote the principles of Personalised Care, including shared-decision making, motivational conversation and patient activation within own practice and encourage colleagues and members of the wider multidisciplinary teams to facilitate a culture of patient-centred care.

Lead a specialist area or pathway within musculoskeletal physiotherapy practice (First Contact Practice in primary care) providing expert advice, education and support within your sub-specialty area to physiotherapy and multi- disciplinary colleagues, patients, carers and third sector organisations.

Provide clinical leadership and mentorship to Band 7 Physiotherapists, Rotational Band 6 and 5 Physiotherapists and Physiotherapy support workers within the specialty through supervision, assessment, education and development including both planned and experiential learning opportunities.

Lead the supervision and assessment of undergraduate/post-graduate Physiotherapy students within Musculoskeletal Rehabilitation as required ensuring the standard of practice and teaching meets the standards set by a degree/higher degree level.

Clinical Practice

· Accept clinical responsibility for a designated or shared caseload of patients being professionally and legally accountable for all aspects of your work including the virtual/telephone/face-to-face management of patients in your care across a variety of settings including primary care, the MSK Services and specialist clinics.

· Using advanced knowledge, skills and training in the management of MSK conditions, act as an expert to deliver specialist musculoskeletal clinics undertaking advanced biopsychosocial assessment of patients including those with complex / multiple pathologies. Use investigative, advanced clinical reasoning and analytical problem solving skills to formulate differential diagnoses. Demonstrate safe, effective, autonomous and reflective practice informed by available evidence and established best practice guidelines.

· Be competent in referring for diagnostic tests (i.e. x-ray, MRI, ultrasound, EMG (studies and blood tests) and be able to competently interpret results as required, communicate outcomes professionally to patients, sometimes delivering difficult news.

· Work in an advanced practitioner role administering advanced treatments safely and effectively according to Trust and Departmental SOPs, maintaining evidence of competence. (i.e. injections, ultrasound scanning).

· Undertake independent prescribing where appropriate, minimising the need for pharmacological intervention and ensuring competence and practice are maintained according to Trust policy.

· Possess specialist clinical knowledge related to musculoskeletal care acquired through Master’s level (or equivalent) study which is supplemented by vast Specialist experience training.

· Act as an expert clinician for musculoskeletal related conditions acting in a consultancy role for other staff. Act as an excellent role model to musculoskeletal colleagues and provide expert advice, support, guidance and education in the field of musculoskeletal physiotherapy.

· To work within CSP, Departmental & Trust Guidelines, National Clinical Guidelines and National Service Frameworks and to monitor own and others standards of practice as appropriate

· To work closely with physiotherapy services, Orthopaedics, Rheumatology and Pain Services to ensure patients are offered an equitable service

· Uphold and safeguard individuals’ interests and provide information on the risks, benefits and potential outcomes of decisions about MSK care to facilitate shared decision making, goal setting and agreement of a personalised care plan.

· Formulate and deliver individual physiotherapy treatment programmes based on advanced clinical knowledge of current practice, evidenced based practice and treatment options

· Make appropriate referrals to other services and clinicians based on the assessment and investigation findings and by having an in depth understanding of the other members in the multidisciplinary team

· Recognise boundaries of own Advanced Clinical practice as it develops and to manage associated clinical risks at all times

· Understands professional accountability and legal frameworks for advanced practice

Learning and Education
• Be responsible for maintaining and developing current knowledge of evidence-based practice ensuring own competency to practice through continuing professional development (CPD) activitie
• Be an active member of the continuing education, in-house programmes by the attendance and presentation at staff meetings, tutorials, training sessions, journal clubs, external courses, clinical supervision and reflective practice
• Undertakes annual mandatory training and ‘essential to job role’ updates and other relevant courses in line with professional and Trust policies
• Develop and provide expert advice, teaching and training to other members of the MDT as appropriate, both internal and external to the trust. This may be formal or informal
• Be responsible and proactive in developing own professional practice, demonstrating evidence of increasing autonomy, clinical judgment and decision making skills and maintain a portfolio that reflects personal development, whilst contributing to the development of others
• Provide supervision, support and training for physiotherapists and foundation advanced clinical practitioners enabling them to develop and apply their knowledge and skills in practice. Provide timely feedback to individual practitioners and their line managers to support this process, identifying, reporting and proactively dealing with poor performance
• Advise, guide and teach less experienced physiotherapists, assistants and students in the assessment and management of patient and other aspects of clinical car
• Promote and support the musculoskeletal physiotherapy profile within Higher Education Institutions, working to ensure pre-registration training reflects current healthcare demands and evidence-based practice.
