Summary
CLINICAl Work as a Health/Social Care Assistant to assist with the coordination and management of patients undergoing treatment at Liverpool Heart and Chest Hospital to support patients both on and off treatment. Provide information to patients related to treatment/follow-up and signpost to appropriate healthcare providers within remit of role and guidance Be the point of contact for all routine enquires facilitating communication and information flows, initiating and responding to correspondence and resolving enquiries. Provide telephone advice and triage where appropriate working within defined protocols and guidelines. Document and communicate outcomes as per service specification. Provide level 1 psychological support within own knowledge boundaries following appropriate training. Perform Holistic Needs Assessments using designated screening tools as appropriat Identify with the Advanced Nurse Specialists/ Cancer Nurse Specialists those patients requiring a Holistic Needs Assessment and support those requiring Level 1-2 psychological support within own skill base. Communicate directly with patients, relatives and carers demonstrating empathy, understanding and respect. Information discussed and received will often require tact or persuasive skills, to ensure safety and understanding around treatment side effects as well as an understanding of complex information around treatment options and end of life care. Support the Advanced Nurse Specialists/ Cancer nurse Specialists in developing and facilitating Health and Well Being Programmes. This will involve contributing and delivering education to patient and carers, including information around health promotion, disease prevention and supporting self-management of their rehabilitation or long term condition. Ensure patients receive timely and appropriate information in regard to their diagnosis and treatment and signpost to appropriate supportive leaflets and literature. Provide support for thoracic surgery cancer patients when receiving cancer diagnosis Provide support in thoracic surgical clinics for patients diagnosed with recurrence of cancer Assist in the discharge planning of patients with appropriate community follow up for their psychological needs or support. Participate in other duties to reflect an evolving healthcare service as appropriate. Support the wider Cancer Services Team during periods of sickness and annual leave. Undertake any other duties which are deemed appropriate to the band. Liaise closely with the MDT co-ordinator ensuring robust systems of notification and where appropriate co-ordinate transfer of care Plan and organise own workload to ensure patient needs are prioritised. Plan and organise own day to day workload, which will also involve liaising with several other healthcare professionals and external agencies. Develop patient care plans, based on information discussed within an Holistic Needs Assessment MANAGERIAL/LEADERSHIP Contribute to the development of MDT/team objectives for the service/speciality. Contribute to local clinical networks and speciality specific groups, working in partnership with other key professionals. Demonstrate the ability to allocate, coordinate, monitor and assess own workload and that of individuals or the service. Ensure appropriate use of resources in order to meet service/clients needs. Identify gaps in service provision and in agreement with the MDT implement changes required to improve service provision. Recognises, understands and accepts the requirement to act as an advocate for patients, carers, service and organisation ORGANISATIONAL Demonstrate a broad understanding of cancer treatments offered within the Dorset Cancer Centre Assist with the preparation of documentation for preparation Cancer Peer Review assessments Participation in obtaining and collating patient satisfaction with care using questionnaires, results of Friends and Family Test and new patient experience tools such as Evidence Based Design Reporting all complaints and untoward occurrences as per trust policy. Support the delivery of information prescription delivery in consultation with health professionals and according to any agreed information pathways, including printing of information. Ensure all documentation is recorded accurately, legibly signed and dated in accordance with Trust policies Responsible for Health & Safety in areas managed, promoting a safe environment for staff and visitors. Maintain safe systems of work, standard operating procedures and risk assessments for area of responsibility. Correctly and safely use equipment and physical environment. Attend all aspects of mandatory training. Ensure adverse occurrences are recorded and reported via the Trust incident reporting system in a timely manner. Conform to health, safety and security legislation, policies, procedures and guidelines. Document all interactions both verbally and written with accuracy and in line with Trust policy and procedure. Demonstrate excellent communication skills both verbal and written. Keep all assessments up to date electronically and maintain database. Ensure accurate and timely documentation. Organise own workload and be able to update Clinical Nurse Specialists of progress in line with agreed work plan. Participation in Macmillan training and education opportunities for self-development and shared experience. Identify personal education needs and skills development with the registered practitioner Be responsible for own personal educational, keeping up to date with changes to practice, to ensure highest possible standards of work. Participate in an annual review with the Cancer Nurse Specialist and Lead Cancer Nurse ensuring highest possible standards of work. Review own work against career framework outline and identify with the relevant people the activities to be undertaken to support learning and development. Actively take part in learning activities and maintain a record of these in personal portfolio in order to obtain skills required for the post. Support CNSs/ANPs and MDT in developing and facilitating Health and Well Being Programmes.
