Summary
We are delighted to provide an opportunity for an experienced band 6 or band 7 respiratory physiotherapist to join our team. We are looking for a dynamic, experienced physiotherapist with a particular interest in Pulmonary Rehabilitation/Integrated Respiratory Care. This is a one year post where you will provide leadership to and work as part of the physiotherapy and wider MDT working in Respiratory Care. As a member of the Integrated Respiratory Team (IRT) you will provide a high standard of physiotherapy assessment, treatment and advice for respiratory patients through the delivery of pulmonary rehabilitation, COPD admission avoidance, early and supported discharge, asthma services, home oxygen services and the COPD care bundle across KHP and the community.
• To take a lead role in delivery of the pulmonary rehabilitation service.
• To lead the advanced assessment and treatment of stable and acutely ill patients with chronic respiratory problems, with multi-pathology including those who have highly complex presentations, and those being assessed for pulmonary rehabilitation. To determine clinical diagnosis and treatment indicated and to maintain associated records, as an autonomous practitioner
• To take a lead role in supervision and teaching of physiotherapists / physiotherapy assistants / students and MDT staff in physiotherapeutic care of patients with respiratory problems.
• To provide skilled team leadership and be responsible for operational management, team development, implementation of policy and policy changes in own clinical area, and to set and monitor standards of practice.
• To supervise, initiate and participate in evidence-based projects, including recommendations for change in practice and to lead in departmental research and audit in their clinical area.

Patient Care
• To be professionally and legally responsible and accountable for all aspects of the practitioner’s professional activities
• To undertake the specialist assessment as an autonomous practitioner, carry out diagnostic procedures and interpret the results for patients with multifaceted respiratory and / or rehabilitation problems in the IRT to the local community, based at St. Thomas’ Hospital and in the community, including those patients with a complex presentation, using investigative and analytical skills.
• To interpret and analyse clinical and non-clinical facts to form accurate diagnoses and prognoses in a wide range of highly complex conditions, to recommend the best course of intervention, and to develop comprehensive discharge plans.
• To formulate, evaluate and reassess individualised physiotherapy management and / or treatment plans, using clinical reasoning skills and utilising a wide range of treatment skills and options in order to formulate a specialised programme of care.
• To be responsible for own and team patient care plans.
• To provide advice on specialised physiotherapy care and treatment for patients with respiratory problems based on theoretical knowledge and relevant practical experience, and to be consulted frequently for specialist advice and guidance by junior team members and other health care professionals both within and outside the trust
• To demonstrate a sound understanding of clinical governance, clinical risk and to demonstrate the use of treatment plans with an evidence-based link.
• To be responsible for maintenance of accurate written records using POMR system and the use of the computerised patient data system, and supervision of team record keeping; to include comprehensive progress and discharge reports
• To demonstrate physical ability to carry out physiotherapy assessment and interventions, including manual physiotherapy techniques and therapeutic handling.
• To ensure that a high standard of clinical practice and patient care is maintained and to ensure that other physiotherapists in the team do likewise, through ongoing competency assessment and evaluation of treatment outcomes.
• To keep up to date with clinical developments, analysing current research and discussing and implementing changes in clinical practice accordingly.
• To be informed of and work to the standards in the specialty area, and ensure that junior members of the team are informed and work to these standards.
• To use specialist knowledge to refer to other health disciplines as appropriate.
• To demonstrate highly developed dexterity, co-ordination and palpatory sensory skills for assessment and manual treatment of patients
• To be highly competent in assessing a wide range of conditions experienced by patients with respiratory problems based on advanced theoretical knowledge of anatomy, physiology and pathology, often in situations where conflicting evidence is present.
• To participate in the on-call and emergency duty rotas as lone practitioner within the Trust. (See Appendix)
• To undertake any other duties as may be deemed necessary, and be prepared to work at any location within the Trust.
