We are looking for a Registered Nurse (Adult – all disciplines), Allied Health equivalent (registered HCPC) or registered Social Worker to join our All-Age Continuing Care Service across the Cheshire area on a permanent basis.

As a Commissioning Practitioner, you will be responsible for assessing individuals, ensuring the National Framework for NHS Continuing Healthcare and Funded-Nursing Care is adhered to. This also includes planning and commissioning packages of care and the review of these to ensure they meet identified health outcomes.

You will support personalised care by offering personal health budgets and help individuals and their families to understand these.

Successful candidates will have excellent communication skills, be resilient and have the ability to work autonomously and as part of a team.

You will play a key role in supporting the award-winning All Age Continuing Care service across Cheshire. We continuously strive for improvement, innovation and have a track record in the development and use of technology to ensure better experience and outcomes for patient and carers.

The post holder will:
• ensure assessments are undertaken in line with the National Framework for Continuing Healthcare and funded-Nursing Care and submitted to the ICB within the service’s key performance indicators.
• commission appropriate packages of care for individuals eligible for CHC or other NHS funded care, in line with the Commissioning Policy.
• provide case management and ongoing review of individuals eligible for CHC or other NHS funded care in accordance with the National Framework for Continuing Healthcare and NHS-Funded Nursing Care.
• manage complaints/disputes.
• process urgent fast track referrals and, where appropriate, procure packages of care in a timely manner for individuals who are receiving or require end of life care.
