Summary


The Neonatal discharge coordinator is responsible for ensuring that each patient has an expected date of discharge (EDD) and that the ward bed status is real time. The Neonatal discharge coordinator is responsible for reviewing all patients on the unit, in terms of ongoing care, management plans and coordinating with the local hospital regarding repatriation. Post holder will attend daily safety huddles with the members of the multidisciplinary team to discuss discharge plans of complex cases and well babies

The Unit is one of three lead centres within South London Neonatal Network and a tertiary referral centre for neonatal surgery. It has a large establishment of 150 nurses and support staff, ranging from Neonatal Assistants to Advanced Neonatal Nurse Practitioners. The unit has a capacity of 40 cots, 12 of which are intensive care, 12 high dependency, and 16 special care. The unit also provides a neonatal community follow-up service.


We are looking for skilled neonatal trained nurse with excellent communication skills and proven evidence of leading and supporting a team of nurses to implement nurse-led discharge, achieving and maintaining appropriate, planned, safe and timely discharge for patient. To coordinate complex discharges and facilitate nurse-led discharge for straightforward patients by engaging the multi-disciplinary team and senior nurses.

You will be expected to work both in Neonatal intensive care and Special Care Baby Unit. Working hours generally are Monday to Friday, however occasional long shifts, weekend and bank holidays might be required.

You must be NMC registered, in possession of a neonatal and equivalent of assessor qualification and have a minimum of 2 years recent experience as a Band 6 or 7.

To provide effective nursing leadership, expertise, advice and support to the NNU team and
ensure the provision of high-quality care. To act as a role model and expert practitioner and to
provide teaching and learning opportunities for junior staff in the care of complex babies examples
include but are not limited to; babies with tracheostomies, on long term ventilation, with stomas,
going home on oxygen or with nasogastric tubes and for discharge on PN.
