Care Home Therapy Team- Fixed term contract until end Sept 2025
Do you have a passion for working with older people living in care homes? From Oct 24 we will pilot a dedicated care home therapy service to help prevent falls in the older person’s residential and nursing homes in Lambeth and Southwark. This is an exciting and rare opportunity to be part of and help establish a small team based in these homes.
You should have an interest in and a commitment to working with frail older people at risk of falls, many of whom have dementia. You should be able to use your excellent communication and inter- personal skills to form productive relationships with care home staff, residents and their families.
You will be required to help facilitate effective working practices in the care homes by providing training, coaching and advice. You will need to demonstrate flexibility, creativity and the ability to support change in a fast-paced environment.
The team will be managed by the friendly and supportive Community Rehab and Falls Service (CRAFS). The role offers opportunities to develop your skills by taking on tasks in the set- up and evaluation of this new service.
1. To work as part of a flexible multidisciplinary team and undertake tasks delegated by senior therapists in a care home setting
2. To contribute to the assessment, planning and review of, and to independently implement rehabilitation programmes with a variety of care home residents. Tasks may include (but are not limited to) supporting strength and balance exercise for falls prevention, practicing transfers and mobility, assisting and promoting independence in personal care tasks, assessment for and provision of equipment to assist with daily activities and documenting interventions accurately in notes following trust guidelines
1. To work collaboratively with care home staff to meet the therapy and falls prevention needs of residents.
1. Support the service's commitment to meeting client needs and delivering excellence. It is essential the post holder be able to work their hours flexibly and contribute fully to both existing and new rotas. This includes 7 day working and non- traditional shift patterns.
Guy's and St Thomas' is comprised of four Clinical Groups (CGs) and Integrated and Specialist Medicine is one of the largest of the groups with a budget of over £500m and approximately 6200 WTE staff.
Integrated and Specialist Medicine consists of 9 Directorates and provides care and treatment to patients who require emergency and inpatient care, community services including district nursing and specialist care, dental services, specialist inpatient / outpatient areas , imaging and pharmacy. Integrated and Specialist Medicine also have responsibility for cross site operations. The integrated approach of the clinical group facilitates and promotes the shared learning of best practice, innovative and creative ways of working to enhance the patient experience and ensure the delivery of outstanding care
The Integrated Local Services directorate within this clinical group provides a range of community and inpatient health services for adults within the communities of Lambeth and Southwark, with some services also provided in Lewisham. Our hospital-based inpatient and outpatient services provide care for adults from across the region.
