Summary


An exciting opportunity has arisen to join a well-established Urology nursing team to care for patients undergoing prostate, bladder, penile, testicular and pelvic sarcoma cancer treatments at The Christie..

Lead on The Christie Prostate easy Access Support Service (PEASS)Providing care and supporting ongoing management of patients who are being and have been treated for prostate cancer at The Christie. The post holder will be expected to develop their scope of professional practice to meet the needs of the service and be expected to take up the key worker role for patients who are suitable to be followed up remotely.



You will be responsible for working with IT, financial, managerial, and clinical experts to maintain a safe and effective PSA monitoring system incorporating stratified follow up guidelines, delivering personalised care, and encouraging a culture of self-management.



The post holder will have the opportunity to develop knowledge, understanding and clinical skills in in the management of patients with urological cancers and there may be opportunities in the future for further role development.

The post holder will be expected to demonstrate sound leadership skills in leading and continuing to develop the service whilst fostering collaborative working relationships, leadership, consultation and education..

You will provide specialist support and advice to patients acting as their Keyworker. You will oversee and co-ordinate care and personalise the patient pathway to deliver a supportive nursing service for patients and their families undergoing in-patient and out-patient treatment at The Christie. You will be responsible for delivering specialist urology cancer care across organisational, professional, and geographical boundaries both autonomously and as part of a highly valued member of the urology surgical oncology team.

The Christie is one of Europe’s leading cancer centres, treating over 60,000 patients a year. We are based in Manchester and serve a population of 3.2 million across Greater Manchester & Cheshire, but as a national specialist around 15% patients are referred to us from other parts of the country.

We provide radiotherapy through one of the largest radiotherapy departments in the world; chemotherapy on site and through 14 other hospitals; highly specialist surgery for complex and rare cancer; and a wide range of support and diagnostic services. We are also an international leader in research, with world first breakthroughs for over 100 years.

We run one of the largest early clinical trial units in Europe with over 300 trials every year. Cancer research in Manchester, most of which is undertaken on the Christie site, has been officially ranked the best in the UK.

DUTIES AND RESPONSIBILITIES

CLINICAL

· Develop and maintain clinical skills in assessment, and knowledge of diagnosis and treatment options for patients with urological cancers.
• Ensure excellent standards of patient care are always maintained.
• Manage a defined urological cancer patient caseload.
• Liaise with other Clinical Nurse Specialists, Cancer support workers, and other professionals, as well as those from referring units, to ensure effective transfer of information across different NHS organizations and smooth pathway of care.
• Ensure the privacy, dignity and safety of individuals is always maintained and ensure that all management of data is carried out with adherence to Data Protection Policies
• Recognize issues relating to safeguarding vulnerable children and adults and report any problems or raise concerns to the appropriate registered care professionals.
• Develop skills in the holistic management of urology cancer treatment related side effects.
• Develop skills in providing complex telephone support and advice, with support from the senior CNS team.
• Collect, collate, evaluate, and report information, maintaining accurate patient records in line with clinical governance and risk management.
• Work independently managing own case load of patients who are on Personalized Stratified Follow Up.
• Develop knowledge and understanding of follow-up pathways and be responsible for managing their disease surveillance and recognizing signs of recurrence.
• Develop advanced skills in managing treatment related side effects.
• Use advanced communication skills in a variety of settings, including discussions around patient care and treatment plans with patients, carers, relatives, and members of the multi-disciplinary team.
• Provide emotional support to patients, their families and staff during difficult situations e.g. breaking bad news or following an unexpected event.
• Involve patients/carers in the planning and delivery of care and development of services to enhance the patient/carer experience.
• Be involved in nurse led clinics.
• Apply evidence-based research to practice.
• Act always in accordance with the NMC code of conduct and guiding documents.

LEADERSHIP
• Play an integral role in the operational and strategic development of prostate cancer after-care services.
• Lead on the personal stratified follow up service at The Christie for Urology.
• To actively contribute to any relevant initiatives within the care group and provide support to colleagues, patients, relatives, and carers.
• To work with and support managers and clinical staff to identify areas where changes in practice are required.
• Ensure effective and efficient use of physical and financial resources.
• Monitor health, safety and security of self and others and promote best practice in the clinical area.
• Participation in the development and implementation of policies, procedures, and guidelines at local and network level.

EDUCATION

· Evaluate learning needs and plan, implement, and evaluate programmes of education to meet identified needs.

· Attend meetings and conferences as appropriate to ensure you are fully conversant with current practices both within the trust and within specialist urology oncology services locally and nationally.

· Present relevant clinical practice, audit, and service developments.

· Participate in the education of pre and post-registration health care professionals/medical staff

· Ensure own compliance with mandatory training.

· Engage with clinical supervision as part of own professional development.

· Maintain up to date skills, knowledge, and awareness of professional issues.

· Participate in planned programmes of appraisal.

PROFESSIONAL DEVELOPMENT

· Develop teaching and presenting skills and apply the principles in the clinical setting.

· Keep up to date with current literature and research within the speciality.

· Maintain own professional development and attend mandatory study sessions as required.

· Actively participate in own performance appraisal.

· Adhere to NMC code of professional conduct accepting and working with appropriate levels of competency.

Research and Audit

· Identifytheneedforandundertakeclinicalaudit and benchmarking, incollaborationwiththeteamtoimproveeffectivenessofpatientcare.

· Disseminateresearch andaudit findingsthrough presentationto professionalgroupsandpublication.

· Contributetothewritingofprotocolsrelevanttoownclinicalspecialty.

· To promote and facilitate evidence-based practice and clinical audit within the Trust, raising the profile and enhancing the contribution of urological nursing.

· To promote nursing research and evidenced based practice relevant to the specialty.
