Summary


The NET (Neuroendocrine Tumour) & Genetic Endocrine Navigator role is integral support to the team, assisting and co-ordinating patients referred to the trust for confirmed cancer or genetic predisposition. You will contact patients at the start of their pathway to ensure they can communicate any concerns they may have regarding their care. You will act as a point of contact, maintaining effective communication with patients referred into the NET and Genetic Endocrine clinics, demonstrating the Trust's values and behaviours. We are looking for a self-driven, highly organised, flexible individual with excellent communication skills who is aware of the cancer pathway and can ensure cancer waiting time standards are achieved and crucially that patients receive their diagnostics and treatment in a timely and efficient way. You will be required to update the Patient Tracking List (PTL) and use Somerset Cancer Record to update the patient's treatment pathway.
Ensure all patients have timely appointments within patient access targets and that the patients are aware of their appointments.

Administration tasks including printing/scanning documents. Managing emails. Telephoning patients to discuss appointments and investigations required.

Responsible for ensuring up-to-date relevant information, including any patient contact is recorded on the Trust's databases including tracking notes on Somerset Cancer Registry (SCR) in advance of weekly PTL and MDT meeting.

Co-ordinate diagnostic investigations, signpost patients to appropriate support and assist with travel arrangements. Ensure the delivery of patient care is of the highest standard and those patients and their families are treated with dignity and respect.

Escalate histological results on suspected cancer specimens via Liverpool Clinical Laboratories (LCL) when required.

Update Patient tracking information on Somerset Cancer Register, add alert if additional support or adjustments are required to support attending appointments e.g. Learning disability, requires interpreter etc

The main responsibilities for this role are articulated in the main duties section.

• Assist clinical team with the referral vetting process (requesting additional bloods, calling GP about rejections, etc.)
• Attend weekly Patient Tracking List (PTL) & Multi-Disciplinary Team Meetings (MDT)
• Chase and escalate diagnostics tests requested by clinician, ensuring correct priority is selected, until the point a cancer has been diagnosed or excluded
• Proactively monitor to identify patients who are likely to breach their waiting time target and escalate appropriately to enable action to be taken to prevent breaches, highlighting problems to clinical team and at PTL meeting
• Negotiate with and instruct relevant personnel, departments of clinic bookings/diagnostic tests/admissions that need to be arranged or bought forward to ensure prompt diagnosis.
• Access the Trust interpreter service when appropriate in accordance with current Trust policies and guidance
• Develop and maintain effective working relationships with Trust wide services, including clinicians, Clinical Nurse Specialist (CNS), MDT Co-Ordinator, other medical staff, medical secretaries/PAs, appointment booking clerks and administrative staff, to ensure all patients on a cancer pathway are identified and tracked.
• Actively follow up those patients who have failed to attend appointments, to achieve improvements in compliance. Offer to arrange support if needed to those who are having difficulty attending.

• Assist the clinical team in coordinating care by tracking patient pathways and providing a point of access.
• Liaise closely with the MDT Co-Ordinator ensuring robust systems of notification when cancer has been identified by the clinical team and the patient needs discussing at MDT.
• Communicate with Patient Access Centre to co-ordinate making OPD appointments for those with abnormal results, fast track patients back into the system if required.
• Co-ordinate notifying patient's GP practice of serious diagnosis on agreed template within 24 hours of patient being informed.
• Support patients and their families to access appropriate information and support, by signposting to a range of clinical and support services.
• Attend all aspects of mandatory training, actively taking part in learning and supportive activities to further develop skills required for the post.
• Take an active part in Care Group Team Meetings as required.
• Provide information to patients and their families on how to make contact when they feel that their condition or needs have changed, including what to do out of hours.
• Ensure smooth handover of patient information to relevant clinical teams following a confirmed diagnosis of cancer.
• Have understanding and confidence of when appropriate to liaise with GP staff/ Cancer Community Support Service who can support with complex patients who may be proving difficult to engage and/or attend hospital appointments.
In addition, the post holder will be able to signpost patients and carers to the relevant sources of support and information appropriate to their needs.

There will also be an expectation of supervisory management of the admin team supporting the Assistant Operational Manager.
