Summary


The Acute Care Team are a 24/7 critical care outreach service who cover the footprint of East Lancashire Hospital Trust and sit within the division of surgery and anaesthetics. The team of ACPs,Non-Medicalprescribing practitioners, and specialist vascular access practitioners with varying backgrounds in both nursing and alliedhealth professions are led by a consultant nurse and lead ACP structure who provide direct clinical supervision and guidance in the workplace.


The role of the ACP is to have daily oversight of the team working across all sites and to both manage and lead the team in the response to deteriorating patients,high riskpresentations,  andrecovery programs. The successful applicant will have an open mind to innovative practice and work towards a level of advanced practice which is evolving andbrakingdown barriers to conventional practices. 


The trainee program will takeanhybrid approach to development within the role and consist of bothworkbased learning and academic study at level 7. This will befacilitatedvia an apprentice route onto the MSc degree program for Advanced Clinical Practice at the University of Central Lancashire where a number of both compulsory and optional modules including clinical decision making, non-medical prescribing and quality improvement will progress the trainee to a level of independent practice.

East Lancashire Hospitals NHS Trust provides acute, community and specialist services to the population of East Lancashire and Lancashire/South Cumbria. 


Services are provided from two main sites, the Royal Blackburn Teaching Hospital and Burnley General Teaching Hospital. Acute services are housed on the Blackburn site and include the emergency department, the 32 bedded Critical Care Unit and the acute surgical and medical units. The ED is a trauma unit in the Lancashire Trauma network.


ROYAL BLACKBURN TEACHING HOSPITAL 


The Royal Blackburn Teaching Hospital provides acute services in 503 +70 daycasebeds   


The hospital is situated just off junction 5 of the M65 and is our main hospital site receiving all emergency patients dealing with allemergencyinpatient care. All blue light ambulances attend this site. 


BURNLEY GENERAL HOSPITAL 


Burnley General Teaching Hospital is an acute District General Hospitallocatedin Burnley, Lancashire, England. The hospitalprovidesa full range of services, including general and specialist medical and surgical services, diagnosticservicesand support services. It also houses an Urgent Care Centre for minor injuries andillnesses.



Personal Profile:

• Candidates must meet essential criteria and be willing to undertake an advanced practice pathway at level 7 master's degree including non-medical prescribing.
• Be flexible and prepared to work across a 24/7 service covering all East Lancashire Hospital Trust (ELHT) sites.
• Exercise advanced clinical expertise, level of judgement, discretion and decision making in clinical care of patients, which may include highly complex medical and surgical presentations.
• Must have experience within an acute setting and have worked at a senior level for a minimum of 3 years.
• Have recognised skills to function at a high level to support the Acute Care Team rota.
• Be prepared to undertake rotation throughout clinical areas to develop required and recognised skill set that support deteriorating patients, including critical care, emergency medicine, and community and integrated care settings.

Role:

• To provide expert advice, support and direction to ensure holistic patient care for acutely unwell and deteriorating patients with the multidisciplinary team.
• Work closely with the critical care nursing and medical teams to facilitate early and timely admissions, safe discharges, and follow up and rehabilitation care.
• Collaborate with the deteriorating patients steering group and division of Education, Research and Innovation to develop and deliver acute illness management education.
• Provide the infrastructure that addresses the knowledge and skills deficits of clinical areas in delivering acute care skills to patients.
• Support the Implementation of national guidelines and / or evidence-based practice to improve patient care.
• Addresses clinical governance issues promptly.
• Provides direct leadership for the Acute Care Team.
• Provide highly specialised care within the hospital setting, to those patients with chronic and acute conditions using advanced clinical knowledge that is evidence based
• Function independently or within a clinical team demonstrating competence complementary to the MDT in the delivery of health care.
• Provide expert advice and ensure the needs of the patient are met by leading, challenging and changing practice within the clinical setting.
• Accept new and follow-up referrals
• Discharge patients from workload as appropriate
• Undertake clinical audit and research
• Recognises and acts as an advocate for patients, carers, service and organisation.
• Lead change as required and ensure resolution of conflict.
• Undertake and lead quality improvement projects to support service development.

General Responsibilities:

• Working as a Trainee Advanced Clinical Practitioner making independent clinical judgements/decisions based on patients' clinical need and within scope of competency
• Undertake independent comprehensive assessment and management of patients to incorporate physiological, psychological and pharmacological considerations, making multifaceted complex clinical decisions regarding patient management and clinical outcomes, including, where appropriate, listing patients for elective procedures and discharging patients from the service when treatment is complete/optimum outcome is achieved
• Demonstrate a high level of competence in clinical, diagnostic and examination skills
• Provide counselling and advocacy for patients with acute and chronic conditions
• Act as a facilitator, resource and model to nursing and medical staff, and professions allied to medicine
• Undertake research and service development
• Regularly re-evaluate the service and develop according to need, within the limitation of resource
• Establish close liaison and communication between all disciplines involved in the care of patients and their families and work in partnership with other practitioners
• Monitor and evaluate the role of the Advanced Clinical Practitioner, liaise with appropriate managers and medical staff, and demonstrate and respond to an evolving service
• Develop and sustain close working relationships, partnerships and networks and draw upon multiagency/inter-professional resources where appropriate to improve health.
• Keep up to date with developments in the role of the Advanced Clinical Practitioner through personal professional development, in service training and education, professional conferences, courses and seminars as appropriate.
