Summary

The Clinical Service Manager is responsible for the strategic, clinical and operational leadership of the Tower Hamlets Coventry Road Pilot project, ensuring delivery of high quality, modern, patient-centered services. This is a pilot project aimed to establish and test out a new model of care that breaks down current barriers to care, is nonrestrictive, co-produced, culturally sensitive and accessible. The service will run 24hrs a day, 7 days a week for people residing in the catchment area. It will offer open access to a range of psycho-social- medical Interventions at every stage of their wellness journey, this includes short term Hospitality stays for those in crisis, and community based offers that are embedded in the neighbourhood. Working collaboratively with residents in PCN One and across VCSEs, social care, primary, mental health and acute services the post holder will support service delivery. This includes working closely with NHSE colleagues, implementing and managing systems and processes to ensure that the service delivers in accordance with the agreed outcomes set out for the service and ethos of the project. The team is multi-disciplinary and multi-agency and committed to co-production, facilitation and liaison with service users, local residents, local VCSE, health and social care services in order that people with co-occuring physical and mental health conditions, experience good outcomes for their mental health. Additionally the team provides specialist input where this is required. The post holder will facilitate the setting up of the project, lead the team, coordinating its specialist and strategic health facilitation roles, and will be expected to manage budgets and balance clinical and operational demands with future planning developments and management of associated change.
