Summary
Key responsibilities and accountabilities: Clinical Plan, co-ordinate, deliver and evaluate patient care and ensure safe discharge of patients in accordance with Pioneer Discharge Policy To provide professional leadership and demonstration of competent advanced practice in tissue viability acting as the lead clinician who supervises staff. Assessing, planning, implementing, overseeing and evaluating programmes of care and advanced skills. Respect peoples dignity, wishes and beliefs; involve them in shared decision-making and obtain their consent before undertaking procedures Promote a patient focused multidisciplinary approach to care in collaboration with colleagues, carers, health care professionals and other agencies To work according to the NMC Code of Professional Conduct and relevant professional guidelines as the Lead clinician for a defined group of patients Communication Core Dimension Use communication skills in order to convey complex and sensitive information effectively to patients, carers and other staff, overcoming any barriers to understanding and providing support during distressing or emotional events. Maintain professional boundaries and working relationships with patients and colleagues. To act as patient/staff advocate through the application of ethical, legal and professional knowledge and skills. Educate patients and their carer(s) as required where a need has been identified. Be an effective communicator both internally within Pioneer and externally with all relevant parties recognising that the role of Nurse Consultant is often a prominent figure head of the organisation. Personal and People Development Develop staff within a defined group via coaching and mentoring. Undertake talent management of clinicians/Healthcare support workers through recruitment, engagement, staff development and performance management. Ensure that all staff attend organisation/local orientation programmes, mandatory training sessions and annual updates. Be responsible for developing and sustaining own knowledge, clinical skills, reflection and appropriate learning from practice. To maintain and develop appropriate competence, performance and professional awareness in accordance with CPD and personal professional profile requirements, this includes being appraised at least annually and contributing to own personal development plan (PDP). Identify the learning needs of key staff within the wider workforce, leading on professional development of the advanced practitioner workforce. Undertake the training, development and competency assessment of all clinical staff and complete assessors training as required. Health, Safety and Security Maintain an open and transparent safety culture by embracing and embedding Freedom to Speak up Principles. Take responsibility for promoting a safe environment and safe patient care by identifying areas of risk and following the Incident, Serious Incidents and Near Misses reporting policy and procedure. This includes identifying potential risks for all staff, patients and visitors and adhering to relevant incident reporting procedures. To ensure compliance with Pioneer policies for Clinical, Corporate, Health and Safety, Information Governance and Human Resources, personal and patient safety, prevention of healthcare-associated infection (HCAI); this includes hand hygiene, use of personal protective equipment and safe disposal of sharps and clinical waste. To ensure that all Pioneer Risk Management systems and processes are implemented to reduce risk to its lowest practicable level; this includes raising risk awareness, seeking risk and safety advice and escalating risks that cannot be managed locally to the Senior Management Team and Pioneer Board. Service Improvement Participate in the development, implementation and monitoring of nursing practice, standards, guidelines, protocols, policies and care pathways. Instrumental in the setting and monitoring of measurable standards of care and be accountable for maintaining standards. Promote and deliver nursing/clinical practice and care delivery in line with relevant current validated research evidence, best practice when available and with reference to Pioneer policies and guidelines. Contribute and manage research and development programmes. Develop and maintain resource management skills which includes taking responsibility for ensuring that Pioneer resources are used efficiently and with minimum wastage. Quality Promote a happy and supportive environment for staff. Maintain own and others awareness of the national, professional and local quality issues relevant to the delivery of nursing services. To monitor and audit standards of all care within all clinical settings as agreed with the Clinical Management Team Providing monthly reports to the Senior Management Team Ensure standards of clinical practice, record keeping and reporting are of a required standard and any issues identified are escalated expediently. Uphold quality initiatives that improve customer care and enhance the interface between staff, patients and visitors. Collect/record and analyse data accurately in accordance with professional guidelines and Pioneer Core Standards. Management and leadership Participate in recruitment and selection of staff after undergoing appropriate training Undertake appraisal of designated staff, addressing personal development plans (PDP) and monitoring/managing performance Contribute to directorate/divisional management processes by being a member of the Clinical Leadership Team Safeguarding: Provision of care to meet health and wellbeing needs All staff must be familiar with, and adhere to, Pioneer child and adult protection procedures and guidelines, in conjunction with the Local Safeguarding childrens board (LSCB) and Adults safeguarding board (ASB) policies and procedures. Ensure that all clinical staff are familiar with their role and responsibility around safeguarding children and adults and to ensure that they have completed training at a level commensurate to their role. Records and reports the interventions consistent with legislation and relevant policies and procedures.
