Summary
The purpose of LeDeR is to identify any potential learning from the care of the person who has died, identify any avoidable factors that may have contributed to a persons death and ways of improving health and social care services for people with Learning Disability in the future, in order to reduce premature deaths. Supported by the Senior Reviewer , the post holder will be responsible for working collaboratively with our provider colleagues (across the health and social care sector) to embed the LeDeR programme into everyday practice. In addition to the LeDeR programme, the post holder will be required at times to support the Quality Team in the achievement of their goals to secure improvements in health outcomes, reduce health inequalities, and improve patient experience. Main Job Responsibilities Function Specific Responsibilities Support the embedding and monitoring of a locally agreed LeDeR process, working collaboratively with colleagues across NHS Trusts and Local Authority to deliver the LeDeR programme. Build and maintain relationships across the health and social care sector. Identify when cases have involvement with other investigators (eg: HM Coroner, Police, Safeguarding, NHS Serious Incident process) and discuss appropriateness of LeDeR review. When applicable support the facilitation of multi-agency reviews of deaths Provide a range of robust LeDeR monitoring reports highlighting areas of good practice and areas of concern. Present reports to the WY ICB Quality Committee when required. Collate findings of reviews and lessons learnt. Play a key role in focussed West Yorkshire ICB led initiatives i.e. Think LD approach To create a culture which values openness, honestly, candour and challenge. To act as champion for people with learning disabilities, ensuring that people with a learning disability and their families and carers are a key part of the LeDeR process. Committed to improving service provision for people with learning disabilities and their families. To uphold organisation policies on the promotion and principles of equality. Key Role Responsibilities: Work on own initiative, taking advice from manager as required; recognises their work is assessed regularly and embraces the positive aspects of this. Provide timely advice, information, or support to internal and external stakeholders around related function, adhering to policies, legislation, governance requirements and best practice guidance. Gain co-operation and agreement from stakeholders, using effective communication skills including a degree of negotiation, persuasion, or motivation skills where appropriate as part of the role. Handle function specific information, which may be sensitive, complex, or confidential and appropriately recording, transferring and/or co-ordinating such information in accordance with the Data Protection Act Make judgements on a range of issues relating to function, using own expertise and within responsibility level. Plan and manage own workload to weekly, monthly, and annual timescales to ensure continuity of service. Produce timely function specific reports, papers and analysis using prolonged concentration and the appropriate computer, keyboard (hardware) and software packages when required. Lead specialist on a specific project or area, deliver outcomes with a managed approach, providing timely updates to managers as required. Use and maintain information systems and databases relevant to function or project. Develop policies and protocols for own area of work, which impact across wider projects, service area and areas outside of function.
