An exciting new opportunity for an experienced Band 3 Community Phlebotomist to join Harrow Integrated Care Service within the District Nursing Team.
The Harrow Integrated Care service provided services to the diverse residents of Harrow. This post will be in one of the 3 Harrow localities.
The post holder will work closely with the team leads, clinicians to develop the service and maintain close working relationships with both acute and community services.

To undertake venepuncture from patients in their own homes. Visit patients in order of priority as listed; there will be a maximum of two fasting patients at the beginning of the list, and one urgent. The phlebotomist will be expected to see up to 10 patients per daily session. To undertake Blood pressure from patients in their own homes.
To maintain Knowledge, skills and attitudes necessary to deliver a quality service that are responsive to the needs of patients
To report any changes in condition of the patient to the Team Leader or in their absence the designated nurse in charge of the caseload.
To document accurate and concise information on all care given to patient in the community health records.
To be aware of Data Protection Act and follow local codes of practice to ensure appropriate action is taken to safeguard confidential information.
To record activity data accurately, ensuring that the deadline for entry on to the computer system is met.
To attend and actively participate in staff and multi –disciplinary locality meetings.
To participate in the orientation and induction of new members of staff and students
To participate in appraisal and personal development plans to identify and agree training needs.
To participate in training as appropriate to personal and team skills development

To contribute to the on-going assessment of a patient by completing the appropriate paperwork for allocated patients, recording and reporting all clinical and behavioural information to the nurse for action and planning purposes.
To enable patients/carers to make most effective use of resources within the service by ensuring they receive up-to-date information sheets, responding to any questions constructively
To pass onto/seek assistance from other colleagues, promptly, any questions that you are unable to resolve
To liaise and refer patients to the MDT team as appropriate in accordance with local procedures.
