Summary


This is an exciting opportunity for an experienced and enthusiastic respiratory nurse to join the Asthma Care Group within the Respiratory Service at the Trust.

The successful candidate will be part of the asthma team and have day to day responsibility, coordination and delivery of the specialist Biologic injectable asthma treatment and care.

Asthma care is an area of significant growth and change; the post will be required to adapt to, and lead these changes as part of the multidisciplinary team and we are therefore looking for an ambitious, well-motivated nurse who has demonstrable, interpersonal and organisational skills. A good understanding and experience of asthma patient care is essential.

To coordinate and deliver injectable asthma services under Patient Groups Directives ensuring patients receive cyclical and continuous therapy as prescribed and directed by the asthma team.

To provide remote one to one support to asthma patients as referred by the asthma team by telephone, email and virtually via attend anywhere.

To facilitate and monitor the management of care for patients with asthma, both day cases and outpatients.

As part of the asthma team, deliver high service levels to asthma patients

To educate and support ambulatory care nurses at Royal Brompton and Harefield sites to deliver injectable asthma services and remote support.

To provide education and support for day case and outpatient asthma patients and nurses; acting as a specialist resource when liaising with and facilitating the best use of all available services to create a sustainable service on both sites.

To assess, plan, implement and evaluate programmes of care for out patients and day cases asthma patients as directed/advised by the team.

To act as an expert resource for patients their families and ambulatory care nurses, giving advice, education and support that is appropriately planned to meet each individual's needs empowering them to manage their treatment regimens.

To act as an expert resource for the Trust in the provision of injectable asthma treatments undertaking the training of others to provide in house services.

To work with ambulatory care staff to ensure clinical time is well used. The post holder is expected to be uniformed and based on the ambulatory care unit. To co-ordinate and review day case patients attending for additional asthma therapies and blood monitoring including injectable steroids, cortisol and synacthen testing in liaison with the asthma team.

To have an active involvement in the collection of clinical information for the national asthma registry.

To facilitate appropriate repatriation process of patients to a local established centre.

To demonstrate a high level of communication skills and act as an advocate for patients and staff.

To work effectively within the multidisciplinary team.

To meet with the Consultant, Matron, Lead Asthma Nurse and clinical teams on a regular basis to feedback and ensure continuity of care for patients, and decide how the nurse role can compliment the patient’s journey.

To develop and maintain clinical skills and knowledge of the speciality of asthma with evidenced based 'best practice' principles.

To mentor post registration and pre-registration students as requested.

To participate in the respiratory medicine clinical governance meetings and undertake informal teaching at the MDT as required.

To supervise other nurses to develop their clinical skills and assess advanced clinical competencies. Acting as a role model and clinical expert for staff providing education as appropriate.

To work alongside the Lead Asthma Nurse in the education, support, and development of the asthma link nurse team as part of the respiratory directorate link nurse programme.

To work in partnership with colleagues, having accountability for decisions made (in accordance with the NMC Code of Professional Conduct).

To develop professional practice through reflection, participation on post registration courses and attendance at meetings and conferences.

To identify and be responsible for personal professional development, in liaison with the Lead Asthma Nurse.
To contribute to developing the quality and availability of asthma information for families and community health care professionals.
To support and work within the asthma team in the introduction of clinical changes.

To keep up to date with new advances, treatment and new developments in the world of asthma research.

To actively contribute to the supporting documentation to develop the asthma service.

To have active involvement in the collection and inputting of clinical data for the purpose of participation in the national asthma registry.

To be responsible for managing stocks and ordering supplies in ambulatory and homecare to ensure consistent service delivery.

To participate promptly and effectively with the investigation of complaints and incidents in line with the Trusts policy and procedures.

To maintain high standards of nursing care within own practice and monitor the standard care for patients requiring admission to the respiratory medical service.

To ensure accuracy of documentation and to maintain an efficient and accurate record system with relevant information in agreement with the team.

To access and utilise patient information through the PAS and EPR systems and local and national databases.

To provide a discharge summary within 24 hrs of patient discharge for all patients reviewed personally or on a nurse led treatment pathway/protocol.

To be aware of waiting time targets.

Undertake training for electronic information systems in place & under development.

To have an active email account and use this as one of the methods of disseminating information to the team.

Role model use of Trust intranet and internet to staff.

To plan, lead and delegate local audit and present as required; ensure findings are disseminated appropriately and recommendations implemented.

To update and maintain local Excel data spread sheets on Biologic therapy.

To maintain an active awareness of clinical and professional developments within asthma and to disseminate that knowledge to all colleagues.

To identify and suggest relevant areas for research which will benefit patient care.

To participate and be actively involved in multidisciplinary research studies of relevance to the care of patients with asthma.

To encourage other staff to recognise the need for medical and nursing research.
To work within the ambulatory care team to ensure all cyclical and continuous treatment for asthma patients is delivered consistently guided by best practice, Trust policies, guidelines and support from colleagues.
To act as a role model and clinical expert when liasing with the local primary healthcare teams, employers and other relevant community health professionals in order to promote a better and on-going understanding of each patient’s condition and associated symptoms and treatment regimes to promote effective continuity of care.

To be actively involved with conferences and meetings, developing presenting skills in the national and international setting.

To participate in the presentation and publication of relevant research and best practice initiatives.
