Summary
An exciting opportunity has arisen to join the dynamic cutaneous lymphoma service in a combined clinical nurse specialist (CNS) nursing and research nursing (RN) role within the cutaneous lymphoma multi-disciplinary team based at the Christie.
The Christie cutaneous lymphoma service is a Supraregional service with an international reputation for providing specialist care and management for patients diagnosed with cutaneous lymphoma, a rare sub-type of lymphoma. The service is always striving to improve patient care and outcomes through service development and clinical trial activity. The post holder will work alongside clinical oncology and dermatology consultants, as well as the established lymphoma clinical research team to provide specialist information, support, co-ordination of care and lead clinical trials for these patients, in both outpatient and inpatient settings. Key responsibilities within the research element of the role would include co-ordination of an agreed portfolio of cutaneous lymphoma clinical trials with the lymphoma research team band 7 nurse.
The post holder will have experience undertaking clinical research. Previous oncology, clinical nurse specialist, dermatology, tissue viability or cutaneous lymphoma experience is desirable. The successful candidate would be supported to develop knowledge, skills and experience any of these areas where required.
The post-holder will be expected to possess excellent communication skills to work as a core member of the supra-regional cutaneous lymphoma team with specific responsibility for the integration of care, providing an expert advisory and support service for patients, carers and other professionals as a key worker for patients throughout their care pathway including beyond treatment.
Alongside the lymphoma research team, the post-holder will lead on a portfolio of clinical trials including all aspects of the planning, conduct and reporting of clinical trials and in-house studies in cutaneous lymphoma. This includes undertaking and maintain proficiency and training in, clinical skills including venepuncture and cannulation, central line care with up to date ANTT skills as well as other appropriate clinical assessment required for the trial protocol delivery.
The post-older will undertake electronic Holistic Needs Assessments (eHNA) and lead and evaluate the electronic Patient Reported Outcomes Measures (ePROMS) in the cutaneous lymphoma service. Excellent organisation skills are required to develop and run nurse led services to meet the needs of the cutaneous lymphoma patient service including skin care management clinics, treatment protocol clinics and skin biopsy clinics.
The cutaneous lymphoma service at the Christie is a busy, dynamic and patient focussed service. The post-holder will join an established multi-disciplinary team including consultant clinical oncologists, dermatologists, histodermapathologists, lymphoma advance practitioner, tissue viability, lymphoma researchers, chemotherapy day services, ECP and bone marrow transplant services. The service is committed to providing the highest quality of care, advancing practice through clinical trials, quality improvement and clinical audit. The post-holder would be integral to service development and delivering a high standard of care for this complex disease.
The Christie is one of Europe’s leading cancer centres, treating over 60,000 patients a year. Clinical Responsibility – patient/service user related tasks/duties
• To act as key worker and central point of contact for patients and their carers referred to the supra-regional cutaneous lymphoma service from the point of referral, throughout their cancer journey. Undertake eHNA at designated time-points to assess individual patient’s need, implement a plan of care, evaluate effectiveness and refer to appropriate services in the acute and community settings.
• To attend cutaneous lymphoma clinics, and provide continuity of care throughout patient’s pathway, particularly with regard to integrating topical, chemotherapy, immunotherapy, radiotherapy and transplantation therapies.
• To attend the supra-regional cutaneous lymphoma MDT meetings, be actively involved in discussions about individual patients and contribute to management recommendations. To attend other relevant meetings as appropriate.
• To assess and provide patients with tailored information about their disease, treatment and management in a format appropriate to their individual needs
• To act as a patient advocate whenever appropriate, with respect for patient confidentiality, privacy and diverse cultural backgrounds and requirements
• To support and counsel patients and carers prior to, during and after treatment including terminal and end of life care. Refer patients to specialist local services, psycho-oncology and enhanced supportive care services when appropriate
• To inform appropriate patients about impact of treatment on fertility, providing advice about contraception and, where appropriate, making referrals to the Regional Fertility Centre at St. Mary’s Hospital.
• To be aware of all cutaneous lymphoma patients admitted as inpatients and liaise with nursing and junior medical staff involved in their care
• To integrate electronic Patient Reported Outcomes Measures (ePROMS) into cutaneous lymphoma practice and evaluate its effectiveness within this patient group.
• To develop and run nurse led services to meet the needs of the cutaneous lymphoma patient service including skin care management clinics, treatment protocol clinics and skin biopsy clinics, adhering to relevant Trust competency requirements including Patient Group Directions (PGDs) for supportive medication required for these clinics.
• To undertake non - medical prescribing within own scope of competence with adherence to relevant Trust policies and current NMC guidance.
• Collaborate with tissue viability/wound care specialists wherever necessary. Advise and, where appropriate, apply dressings and topical skin directed treatment.
• Liaise with NHSBT apheresis team to co-ordinate extracorporeal photopheresis (ECP) services for cutaneous lymphoma patients.
• To be fully conversant with cutaneous lymphoma treatment protocols and co-ordinate the admission of patients requiring inpatient chemotherapy or radiotherapy.
Clinical research responsibilities
• Co-ordinate a portfolio of cutaneous lymphoma trials, ensuring recruitment targets are met. Organise appropriate appointments and manage own caseload of patients according to the requirements of the trial protocols.
• Identify patients suitable for entry into cutaneous lymphoma clinical trials by attending cutaneous lymphoma clinics and MDT meetings.
• Assist in evaluating /establishing patients’ eligibility for studies through co-ordination and interpretation of screening tests and results. Arrange appropriate appointments for patients according to the requirements of the trial protocol, including co-ordinating special tests in other departments, collection of data and data entry.
• Provide comprehensive nursing assessment of patients at each trial visit, liaising with appropriate colleagues to deal with medical or social problems and non - trial related issues. Also to plan, implement, and evaluate care delivery using advanced clinical knowledge and skills.
• Administer a variety of therapeutic drugs within the realms of the clinical trial protocol, taking into account toxicities and dose modifications. Attend mandatory updates for chemotherapy skills.
• Undertake, and maintain proficiency and training in, clinical skills including venepuncture and cannulation, central line care with up to date ANTT skills, vital signs recording and interpretation as well as any other appropriate clinical assessment required for the trial protocol.
• Collection, processing and shipping of biological specimens according to protocol requirements, and follow-up appropriately on alert values.
• Comply with relevant legislation including EU Directive on Clinical Trials 2004, Data Protection Act 1998 and Computer Misuse Act 1990
• Responsible for accurate, timely and legible completion of all trial and non-trial documentation. Responsible for the collection, co-ordination and computerisation of data generated from clinical trials, to ensure that all data is collected and managed effectively and accurately.
Education and Development responsibilities – development of self and others
• To accept personal responsibilities for own development and professional growth
• To educate patients, families and carers at an appropriate level about cutaneous lymphoma, including treatment side effects and symptom management, to enable self-care wherever possible.
• To provide specialist support, education and training to other healthcare professionals, within and outside of both the Christie, to develop their skills to deliver appropriate specialist care to patients with cutaneous lymphoma.To provide specialist input on relevant post registration courses.
• To present audit, research, quality improvement and service development projects to a local, regional, national/international audience. Represent the supra-regional cutaneous lymphoma service at formal education forums/conferences
• To assess own educational needs and keep up to date with nursing and clinical updates in line with NMC Code. Maintain professional portfolio and meet the requirements of NMC revalidation.
• To formulate a personal development plan and undergo regular appraisals with line manager. To undertake identified training to provide the necessary skills in specialist nursing care of cutaneous lymphoma patients.
• To use current research evidence to develop protocols and nurse led services for cutaneous lymphoma patient group.
• To collaborate with lymphoma and dermatology teams to identify and participate in nurse led research projects relevant to improving the care and experience for patients with cutaneous lymphoma and/or their families/carers. Publish outcomes.
• Provide and receive clinical supervision and mentorship to facilitate reflective practice.
• Promote patient and public involvement activities, leading to service improvement. Engage with lymphoma patient organisations such as Lymphoma Action.
Management and Leadership responsibility – including human resources, financial and other resources
• To act as a leader and role model and demonstrate expert knowledge and high standards of clinical practice to other healthcare professionals locally, regionally, nationally and internationally whenever appropriate.
• To provide specialist, expert clinical knowledge to clinical colleagues, patients and carers/relatives both within and outside the Christie. Support and educate primary, secondary and tertiary care teams regarding individual cutaneous lymphoma patient’s condition and management plan.
• To develop collaborative working with primary care and secondary care services to enhance partnership working. Take an active role in relevant local, regional and national specialist groups.
• Initiate and support clinical developments/projects to promote excellence in patient care.
• Evaluate service delivery; identify areas for improvement and initiate change.
• When required, write a report for the supra-regional cutaneous lymphoma nursing service, circulating appropriately.
• Develop and maintain links with relevant charity and patient groups including Lymphoma Action.
• Promote patient and public involvement activities leading to service improvement, including establishing regional cutaneous lymphoma support group.
• To take full responsibility for own workload and manage own caseload efficiently, effectively and safely, with flexibility to meet the demands of the lymphoma patient service. To identify and maintain supportive networks for self and other colleagues.
• To be fully aware of NHS England cancer targets, NICE improving Outcomes Guidance for Haematological Cancers as well as Trust strategic and directorate objectives. In conjunction with multi-disciplinary team use the available resources to meet these targets.
Professional responsibilities
• Ensures accurate, concise and timely documentation of all clinical contact in electronic patient records in accordance with Trust and current NMC guidance.
• To be fully conversant with supra-regional cutaneous lymphoma evidence-based policies and practices and those of the Trusts relevant to cutaneous lymphoma patient care and contribute to development of new protocols and policies.
• To act always in accordance with the NMC Code and guiding documents.
• Work with service colleagues to continually appraise own performance and achieve the targets set by the NCRN and Trust performance indicators
• To keep up to date with Trust mandatory training requirements for this role.
Research/ Quality Improvement /Audit responsibilities
• To facilitate a high quality specialist service through clinical audit/quality improvement/research projects, monitoring of standards against guidelines and through other quality initiatives agreed by the supra-regional cutaneous lymphoma service
• To use information systems such as electronic patient records to aid regular audit of the clinical role and provide activity analysis as required. Adhere to GDPR requirements.
• To contribute to the National Cancer Peer Review process as a core member of the supra-regional cutaneous lymphoma service.
