Summary
To work as an expert practitioner in the field of sickle cell and effectively coordinate and deliver an expert nursing service for patients aged 14-24 years with a diagnosis of Sickle Cell and Thalassaemia To manage a caseload of patients/clients through the process of transition from Paediatric to Adult Haematology Services To ensure that patients and their families are fully informed and supported by providing clinical and social support including the running of nurse-led clinics To lead in the development of evidence-based clinical nursing practice in line with current research and guidelines To teach/train healthcare teams at the Trust around transition and haemoglobinopathies To establish clear lines of communication within the specialty team to ensure cohesive multidisciplinary management of patients Provide line management to the adult Haemoglobinopathies Clinical Nurse Specialist team and HCC Practice development Nurse To link with healthcare teams in order to increase knowledge and facilitate the care of this group of people in the community or in their local healthcare setting. National Haemoglobinopathies Panel Acute Lead Nurse representative role To liaise with Roald Dahls Marvellous Childrens Charity, the organisation who established this role, by providing an annual audit report and case stories, attending their conferences and workshops, mentoring other Roald Dahl Specialist Nurses and collaborating with the charity and Roald Dahl nursing teams on service improvement and innovation projects in the area of transition and haemoglobinopathies.
