Summary
Provide a high quality one-stop shop diabetes service to patients in the community setting by delivering all 9 annual healthcare checks to patients with diabetes, incorporating phlebotomy, blood pressure monitoring, BMI, foot screening, eye assessment, urine testing and smoking advice.
