We are looking for a highly motivated, enthusiastic, friendly and flexible Band 6 Speech and Language Therapists to join our Community Stroke- and Neurological Therapy Services.
You will be working within an interdisciplinary team who are passionate about supporting adults with stroke, and other neurological conditions, to achieve their goals.
As part of the team you will have responsibility for your own caseload, therefore good time management skills. The ability to work well in a team, as well as independently, is essential. You will support and provide supervision to more junior Speech and Language Therapists, students and support staff/rehab assistants.
You will have the opportunity to work across all areas of the stroke and neurological conditions pathways, and benefit from training and development opportunities offered by the service. You will be encouraged and supported to take part in service development projects, as appropriate. There are well-established team meetings and CPD is encouraged.
You will need to be a car driver as patients are treated in a variety of settings, including their own homes.
To provide a specialist Speech and Language Therapy service to diverse patients, recently diagnosed with a stroke or other neurological conditions, in a range of community settings, using highly developed specialist practice, underpinned by theoretical knowledge.
To manage complex caseloads independently, providing holistic swallowing and communication interventions to patients within the Community Stroke Team, Early Supported Discharge Team and Adult Neurological SLT Teams, as required.
To be a specialist resource of information for health and social care staff, patients, families, carers and external agencies, who may require advice and support and guidance in the management of stroke and neurological conditions.
To transfer individual patients’ ongoing care to other agencies, as appropriate, and ensure that the service provided is conducted in a professional, efficient and effective way.
To meet the service specification objectives and key performance indicators, as part of a multi-disciplinary team, providing high quality stroke and neurological services, through the areas of Early Supported Discharge and Longer Term Rehabilitation.

• Be responsible, as an independent autonomous practitioner, to undertake a significant and complex clinical caseload. Caseload management will include prioritisation and contributing to assessment of new referrals within 72 hours.
• Communicate effectively with all patients, carers and other agencies involved in patient care.
• Use highly developed communication skills to explain complex condition related information from assessment/therapy plans to clients/carers, where there may be barriers to understanding.
• Provide a comprehensive specialist assessment of patients who are referred to rehabilitation services, often with complex presentations, in a holistic manner, using investigative and analytical skills.
• Apply advanced clinical reasoning to devise effective rehabilitation treatment plans, seeking guidance from the senior SLT where applicable.
• Delegate and evaluate treatment programmes to the appropriate team members/ rehabilitation assistants.
• Implement effective, evidence based treatment using a variety of treatment approaches, and evaluation of treatment using standardised outcome measures.
• Maintain accurate and comprehensive records of all activity. Record patient documentation on the RiO system using a mobile laptop device.
• Be professionally and legally responsible and accountable for all aspects of your professional activities working within codes of practice and professional guidelines.
• Demonstrate physical ability to carry out SLT assessment and interventions.
• Respond to unpredictable work patterns where there is a requirement for the post holder to concentrate at high levels, for frequent periods throughout the shift, both during patient contact and when dealing with other elements of the role.
• Provide clinical supervision and facilitate reflective practice to junior team members and student SLTs.
• Support SLTs, Rehabilitation Assistants and student SLTs with caseload allocation and management.
• Liaise effectively with all members of the multi-disciplinary team and work effectively as part of this team, acknowledging each discipline's roles and responsibilities.
• Support senior members of the team with audit and service development activities, where appropriate.
• Keep up to date with clinical developments and maintain background knowledge in the specialities of stroke and other neurological conditions, analysing current research and discussing and implementing changes in clinical practice.
• Attend and contribute to in-service training programmes and team meetings.
• Be responsible for the safe use of equipment used in carrying out Speech & Language Therapy.
• Take an active role in screening all referrals, accepting all those which are appropriate and transferring/returning any that do not meet the criteria.
• Participate in multi-disciplinary team meetings; this will include chairing the meetings when required.
• Complete care plans for patients requiring enablement on discharge from hospital. This will include identifying the number and purposes of care calls that are required and to evaluate on a weekly basis.
• Work towards achieving all local stroke specific competencies required within the role.
• Support the integration of local NHS services, local authority and third sector.
• Participate in the development of appropriate guidelines, protocols, audit & evaluation.
• Identify any gaps in the service and report to the Team Leader/Stroke and Neurological Services Manager.
• Monitor current service provision, and report performance against agreed key indicators.
• Work proactively in seeking the views of others about the service provided.
• Evaluate the service against both national and local performance indicators.
• Ensure that service delivery is in keeping with evidence based guidance and other policy targets.
• Identify need, plan, devise, deliver and evaluate training programmes with a specialised content.
• Organise location, hospitality and materials for training.
• Share specialist knowledge with colleagues at a local level i.e. Multidisciplinary Team Meeting.
• Act as student supervisor/assessor under the guidance of the student co-ordinator, and provide observation sessions.
• Identify and access appropriate Continuous Professional Development at least to a level to meet the Health and Care Professions Council requirements and practise reflective learning.
