Summary
The WHH Frailty Hub pathway works on a Home First philosophy with a focus, where appropriate, to return patients to their usual place of residence or onward destination, on the same day. As part of the specialist multidisciplinary team you will be essential in delivering a Comprehensive Geriatric Assessment, coupled with access to timely diagnostics, coordinated from a dedicated purpose-built Frailty Assessment Unit. The outcome of this assessment will result in the development of a Care and Support Plan, developed collaboratively between the specialist teams with the patient and their carers. This CSP will focus on the persons assets and strengths, as well as identifying areas where they may need support from health and social care professionals.
