Summary


If you have been working as a Band 5 Dietitian with an interest and some experience working with adults who have diabetes and are looking to move to the next step in your career this part time role might be for you.

The successful candidate will join our team of 28 staff (dietitians, a nutrition support nurse, assistant dietitian practitioners and an administrative assistant) who are based at Chesterfield Royal Hospital.

The Trust supports remote working and working from home for all team members as appropriate to role and services.

The post holder will work alongside our full time Diabetes Dietitian and will be a valued member of our diabetes team providing mainly outpatient services which will include MDT insulin pump clinics and gestational diabetes clinics.

Provision on nutrition anddietetic servicesto adults with diabetes.

Clinical.

1. To provide a client centred nutrition & dietetic service at various locations according to patient’s needs.

2. To provide the specialist nutrition and dietetic service provision to the adult diabetes clinics as required.

3. To provide group education to adults with diabetes.

4. To participate in the team approach to the care of individuals referred to the service.

5. To attend the multi-disciplinary adult diabetes meetings as required.

6. To liaise with the wider multi-disciplinary teams within the trust and the health community to ensure the dietetic needs of clients can be met.

7. To accurately collect relevant activity data required for service monitoring, management and evaluation.

8. To develop evidence based dietary information in conjunction with other members of the nutrition and dietetic team.

9. To provide cover for senior dietetic colleagues as appropriate.

10. To work with other members of the nutrition & dietetic team and catering staff to develop menus to meet the need of inpatients at Chesterfield Royal.





Quality Governance.

1. To be actively involved in the implementation and development of Clinical Governance.

2. To advise the Acute Team Leader and the Head of Nutrition & Dietetics on professional issues relating to adult diabetes.

3. To work with dietetic colleagues to ensure the nutrition and dietetic training needs of nursing, medical and other staff is met within the Trust and the North Derbyshire health community.

4. To contribute with other members of the nutrition and dietetic team to the identification, development and implementation of standards of dietetic practice.

5. To actively participate in clinical audit and research.

6. To be involved with mealtime audits and attend relevant catering meetings.

7. To calculate nutritional content of hospital menus for the purposes of dietary assessment, menu coding and meeting national standards.

8. To demonstrate a commitment to CPD by maintaining an up to date professional portfolio.

9. To provide support and supervision for dietetic students on practice placement.

Operational Management.

1. To work with senior colleagues within the team to ensure cover arrangements are in place and patients are seen based on clinical need.

2. To manage nutrition and dietetic service staff.
