Summary


• To provide a high quality of care to a diverse range of Podiatric patients



• To work in collaboration with a team of Podiatrists and other care providers to ensure seamless care delivery for all patients.



• To monitor and supervise services delivered by support staff including Podiatry Assistant, Graduate Podiatrist and Student Placements



• To be flexible and be able to cope with a busy, varied and demanding workload, where it is essential to move between sessional clinics on foot / by transport and to be able to multitask using continued diagnostic and analytical skills throughout the day

• To autonomously assess and diagnose patients with a diverse range of medical and podiatric conditions, using appropriate diagnostic techniques, skills and tools.



• To create and implement tailored treatment and specialist care plans (e.g. ulcer management), using a variety of appropriate treatment options for a range of patient groups including those in the “high risk” and “at risk” categories, focusing on the management of primary foot pathologies including patients with the following conditions:



- Diabetes

- Chronic Neuropathy

- Chronic Disease e.g. Rheumatoid Arthritis

- Wound Management/Tissue Viability Problems

- Terminally ill patients

- HIV



• To adopt a problem solving approach, engaging patients and carers in their treatment plan, negotiating on agreed outcomes, gaining consent and educating users on self-care.



• To provide general and specialist foot health advice, including the care of the “at risk and high risk foot”; providing information on the signs and symptoms of pedal manifestations of systemic chronic and acute diseases.



• To evaluate and review treatment outcomes and implement discharge policies for those patients

meeting the discharge criteria in accordance with the department’s criteria for care policy.



Central London Community Healthcare (CLCH) is one of the largest community healthcare organisations in London and Hertfordshire, providing our services to diverse communities/boroughs in 11 London Boroughs - Barnet, Brent, Ealing, Hammersmith & Fulham, Harrow, Hounslow, Kensington and Chelsea, Merton, Richmond, Wandsworth, Westminster - and Hertfordshire.

• To ensure effective communication of complex, highly sensitive and emotional information to Diabetic patients with limb threatening conditions.



• To liaise with and provide feedback to referrers to the service by letter and/or verbal communication, ensuring that assessment, diagnostic results and treatment plans are understood by the referrer in order that the treatment aims are understood and supported by the wider health care network.
