Summary
An exciting opportunity has risen for enthusiastic and motivated individuals to join our Reactive Emergency Assessment Community Team (REACT) as a senior Registered Triage Nurse.
The REACT team is a multi-disciplinary integrated community service within ESNEFT incorporating, Urgent Community Response service, OPAT (Outpatient Parenteral Antibiotics Therapy), OOHs (Out of Hours) and Virtual Ward pathways patients. This role will be working in the crisis team triaging referrals for our Urgent Community Response Team which has developed pathways with East of England Ambulance Service and the Frailty Virtual Ward, two new and exciting government initiatives.

The role will be office-based working 34.5 per week, weekdays, across a 4-day week Monday to Friday.
To act as a single point of contact for new referrals to the REACT service, providing a telephone triage service to determine priorities of need, intervention, coordinating onward referrals and signposting where required.
To be the first point of contact for all REACT referrals. Referral are received from Consultants, GPs, Hospital Specialist Clinics, DNs, 111, Paramedics, DIST, Social Workers, relatives, carers and self-referrals.
To carry out a telephone triage assessment of the referral through excellent listening, critical thinking, good clinical judgement, and assessment skills
To take a whole system approach working to appropriately allocate or direct all referrals according to relayed information/ capacity and availability.
Continuously monitor capacity within the service. Identifying discharges and highlighting delays.
To participate as a dynamic and innovative member of the multi-disciplinary team.
To prioritise incoming referrals on a case-by-case basis and allocate to the team clinicians as able.
To promote and maintain excellent standards of care within the clinical governance framework
Excellent communication is required to ensure the best possible experience and outcomes for the crisis patients. This includes the ability to conduct excellent handovers, escalate and present patient cases within the Virtual Ward MDT.
