Summary
1 To manage, monitor and deliver a high performing, productive, quality service that is cost effective and work closely with key partners with an increased focus on personalisation and self-directed support, assuring high professional standards of social work practice. 2 To oversee the more complex caseload to include service users who are delayed in their care and have identified barriers to discharge, working with a high degree of personal and professional accountability and autonomy. 3 To act as the primary management contact for the Local Authorities and other external partners in respect of social work in the service, taking a lead role in facilitating relationship development. Support the patients journey and transition from inpatient care to successful reintegration into the community, promoting the service user voice and ensuring that all policies and protocols are followed
