Summary
1.MAIN DUTIES AND RESPONSIBILITIES Patient Care and Assessment: Conduct thorough assessments of patients' health conditions, including physical examinations, medical histories, and symptom evaluation. Order and interpret diagnostic tests and investigations. Develop and implement evidence-based treatment plans for patients, including prescribing medications, administering treatments, and providing health education. Collaboration and Consultation: Collaborate with the general practitioner (GP) and other healthcare professionals to manage complex or challenging cases. Provide consultations to patients, offering advice, information, and support to promote self-management of conditions. Participate in multidisciplinary team meetings, contributing expertise and insights to optimize patient outcomes. Health Promotion and Education: Educate patients and their families about health promotion, disease prevention, and self-care techniques. Provide guidance on managing chronic conditions and promoting a healthy lifestyle. Offer counselling and support for emotional well-being, mental health concerns, and lifestyle modifications. Documentation and Record-Keeping: Maintain accurate and up-to-date electronic health records for all patient interactions, including home visits. Ensure adherence to data protection and patient confidentiality regulations. Document interventions, treatment plans, and outcomes to facilitate effective communication and continuity of care. Professional Development and Quality Improvement: Stay updated with the latest evidence-based organisations, guidelines, and regulatory requirements in primary care. Engage in continuous professional development activities, such as attending conferences, workshops, and relevant training programs. Contribute to quality improvement initiatives within the organisation, suggesting and implementing strategies to enhance patient care.
