Summary
To provide navigation and support to patients diagnosed with a rare or rapidly progressing neurological condition (Motor Neurone Disease, Huntingtons Disease, Cortical Basal Degeneration, Multi-system Atrophy, Progressive Supra-nuclear Palsy). Working as an active member of the Community neuro team the postholder will provide a specialised initial assessment, provide and co-ordinate information, support services, advice and guidance for these patients from new diagnosis to end of life. To maintain and develop relationships and pathways with the other key providers for this patient group including palliative care, charitable associations, acute providers, social care and other HCT providers.
