Summary


An exciting opportunity has arisen within the Medical Care Group, for an for an experienced leader with a proven track record of operational and clinical experience to join the leadership team. managing the UHD Virtual Ward programme.

Our Virtual Ward programme is an evolving service delivering acute assessment, management and monitoring within the community setting, across a range of specialities.

The post holder must possess strong clinical and operational leadership skills, and be able to identify and drive change. If you feel you have these qualities then we would like to hear from you.

Base Location: Bournemouth with cross site working

To be responsible for managing the Virtual Ward to a
specified range of specialities within the Trust, and provide clinical,
managerial and professional leadership to the Virtual Ward team.

To ensure that optimal quality patient care is provided, and nursing and health care standards are upheld.

To ensure that the patient experience is of the highest quality, inspiring
patient and public confidence.

To support the development and expansion of our Virtual Ward service, leading on pathway development, virtual ward capacity and identifying innovation.

To lead and evaluate test of change and to utilise Patient First QI methodology in driving the pathway development and service provision.

To support the Deputy Director of Operations in developing the operational direction of the pathway services.

Actively support the UHD Values and beliefs and share responsibility for the provision of the Pathway model services.

Work collaboratively with colleagues and managers to develop partnerships within the programme ensuring a co-ordinated approach in the delivery of the services/pathways.

The matron role is responsible for delivering the Trust's Strategy and objectives, within their area of responsibility including:
- Ensuring the standardisation of services across all relevant areas to the best possible level,
so that all patients experience equally high quality care;
- Supporting the planning and managing services to ensure the best possible use of
resources;
- Supporting the development of a sustainable safe workforce that is fit for the future within
area of responsibility;
- Ensuring services are delivered in a culture where patient safety, high clinical quality,
service improvement and financial accountability are paramount;
- Ensure the safe, efficient and effective delivery of care across the virtual ward.
