Summary
Carry out structured medication reviews on patients with complex polypharmacy, implementing prescribing decisions and requesting appropriate tests. Reconcile medicines following hospital discharge and work with patients and the community pharmacists to ensure that the patients receive their medication post discharge. Reconcile medicines from clinic letters with the support from the pharmacy technicians. Support long term condition reviews Care home ward rounds to carry out medication reviews and de-prescribe as appropriate. Contribute to reductions in medicine related hospital admissions and re-admissions by supporting patients to get the best outcomes from their medicines. Identify and address medicine related issues. Act as the interface with community and hospital colleagues. Manage patients and healthcare professional medicine queries. In collaboration with the management team Investigate any complaints involving medicine issues. Increase patient quality and patient safety of prescribing through mechanisms such as audit and PDSA cycles. The share the findings with the wider team. Contribute to service improvement within enhanced services QOF and IIF Implement drug withdrawals and alerts eg MHRA Work with the primary care professional and patients to implement NICE and other evidence based guidelines. Contribute to multi-morbidity reviews and multidisciplinary reviews. Provide medicine related information and training to practice staff. Act as a source of medicine information for all of the practice team and patients eg around supplies, doses, side effects.
