Summary
We have a exciting opportunity to join our Swan End of Life & Bereavement Care Team
We have the opportunity for a highly motivated, experience and clinically skilled senior nurse/AHP's with recent community experience to join our Swan End of Life & Bereavement Support Service.
The Swan Service was established at the start of March 2021 providing a seven day service. We are delighted to be able to expand our existing service to provide support patients and loved ones in the community. The post holder will be joining our existing team, to provide expertise in community care and services.
Our team also consists of a Swan Bereavement Midwife, Specialist Nurse and also Swan Children's Bereavement Nurse
The team will report to and support the Clinical Lead and ensure the Swan Model is embedded across the organisation working alongside the end of life, palliative care, chaplaincy and bereavement services.
"The Swan model of care is about providing excellent, individualised end of life and bereavement care for every patient and every family, every time.
It is patient and family focussed and centres on meeting the unique needs of each individual and their loved ones - we have one chance to get it right and this should be every time for every patient and their loved ones".
The post holder will be responsible for developing the relationships and services in the community. They will responsible for embedding the Swan Model, providing bereavement support and EOL education and training to the community.
The post holder will be expected to exercise a high degree of professional autonomy, make clinical judgements using advanced knowledge and skills in order to provide the highest standard of holistic patient care, whilst developing and practicing within clinical protocols and guidelines.
Blackpool Teaching Hospitals NHS Foundation Trust is situated on the west coast of Lancashire, and offers a full range of district hospital services and community health services to a population of 1.6 million in Lancashire and South Cumbria.
The Trust provides services to the 440,000 residents of Blackpool, Fylde & Wyre and North Lancashire, as well as specialist tertiary care for Cardiac and Haematology services across the wider region.

Do you have a good understanding of the national and local agenda in relation to end of life care in both primary and secondary care and have significant experience in end of life care?
Do you have effective leadership skills and driven and motivated to play a pivotal role in service improvement and developing and delivering necessary education within the Trust to ensure all staff are supported to reach fundamental levels of knowledge and skills in relation to end of life care.
