Summary
We are seeking dedicated and passionate Diabetes Specialist Nurses, whether experienced or aspiring to become a diabetes specialist nurse, to join our esteemed Inner London Diabetes Service. Our service spans the London tri-boroughs of Hammersmith & Fulham, Kensington & Chelsea, and Westminster, delivering comprehensive diabetes care.
As part of our multidisciplinary team, you will collaborate with consultants, specialist nurses, dietitians, and administrative staff. You will also work closely with general practitioners and other healthcare professionals, providing tailored support and expert guidance to our patients.
Our main office is conveniently located at Lisson Grove, and we operate clinics at nine different sites, ensuring widespread access to our services. Join us in making a difference in diabetes care and support across Inner London.
- Relevant post registration training and experience in the management of patients with mainly Type 2 Diabetes
- To work closely with the Team Leader within the Community Diabetes Service to provide a specialist clinical Nursing Diabetes Service, which contributes to the strategic planning and delivery of the multidisciplinary Community Diabetes Service in line with the strategic direction of the Service and Trust objectives.
- Ideally be an Independent Nurse Prescriber or demonstrate a willingness to achieve this qualification
- Be a great a team player who is willing to work collaboratively with the multi-disciplinary team to deliver the best care possible for our patients
- Have a strong commitment to the delivery of high quality, safe, patient care and be a good role model in reflecting the Trusts core values
- You should be able to work effectively in an environment with frequent interruptions and within stressful, unpredictable situations including responding to and solving urgent and emergency situations on a daily basis.

§ To be a key member of the multidisciplinary Community Diabetes Service to plan and deliver high quality integrated diabetes care in the community.
§ To provide specialist assessment and treatment of patients with diabetes, liaise with other agencies and health professionals and maintain accurate clinical records.
§ To be responsible for project managing the planning, establishment and continued support of specialist nursing interventions.
§ To be responsible for organising and delivering diabetes education programme for health professionals.
§ To utilise project management skills and work in partnership with service users, other healthcare providers and professionals toplan, develop, implement and evaluate service developments and diabetes interventions based on current evidence-based guidelines and policies, which respond to local need, Trust strategic objectives, government initiatives and national policies.
§ To actively contribute to the development and implementation of nursing clinical standards, policies and procedures for the multidisciplinary Community Diabetes Service.
§ To take responsibility of clinical audits and/or research projects, as directed by the Service Lead and to present reports to the appropriate groups within the Trust.
§ Develop and improve professional expertise via continuous professional development.
CLINICAL DUTIES
§ To be responsible for providing a specialist clinical diabetes service to clients with diabetes on oral hypoglycaemic agents, insulin or insulin conversion and other new therapies, including appropriate counselling and lifestyle advice in various community settings such as GP practices, health centres and client’s homes.
§ To work autonomously to provide expert advice to clients, carers and clinicians. This will involve assessment and the development of treatment plans using evidence-based outcomes and involving patients throughout the process.
§ To use professional judgement and clinical expertise to independently interpret and analyse complex situations and information to formulate treatment plans according to evidence based practice and modify according to changes in clinical condition.
§ To be clinically competent to give specialist diabetes advice, treatment and counselling to support behavioural change, in adults with complex and diverse healthcare needs across health and social care.
§ To provide expert advice to medical practitioners in virtual clinics, by email or phone queries.
§ To respond to urgent referrals and undertake a triage function in partnership with GP’s and secondary care services.
§ To initiate and participate in multi-disciplinary/multi-agency collaborative working to promote continuity of care at the multi-disciplinary Community Diabetes team meetings and case conferences.
§ To use clinical judgment to refer clients to other services, professionals and Social Services when appropriate.
§ To provide timely and informative client reports and discharge summaries for GPs, consultants, allied Health Professionals and other staff.
§ To communicate highly complex and sensitive information in an understandable form to clients, carers and families from a wide range of backgrounds to gain consent, ensure understanding of condition, treatment options, risks, acceptance and compliance with dietary treatment, which may include unwelcome or difficult to accept information e.g. long term adherence to a medication, conversion to insulin.
§ To use highly developed negotiation, persuasion, motivation and counselling skills to facilitate acceptance, behaviour change and ongoing compliance with treatment programmes taking into account emotional, physical, social and psychological barriers as well as strong influences on change e.g. depression, language difficulties, impaired cognitive function, resistant attitudes and religion.
§ To be ultimately responsible for managing a specialist clinical caseload.
§ To be professionally and legally accountable for all aspects of own work and to ensure that own work is within professional standards and clinical guidelines.
§ To be responsible for always complying with legal and professional confidentiality guidelines.
