Summary

Main Duties and Responsibilities To ensure the safe and timely discharge of patients for home IV therapy Demonstrate awareness of legislation regarding IV therapy and vascular access devices and deliver care and advice as appropriate Practice in accordance with the NMC code in order that a safe and quality service is provided Promote the interface between hospital and community to provide a seamless service for patients discharged home on IV therapy Develop good working relationships with departments to ensure the patient journey is effective and efficient Participate in the implementation and delivery of the standards set within the Trust Strategic Objectives Contribute to clinical governance processes including adverse incident reporting Ensure competence in the correct use of all equipment used in the clinical setting. Reporting any faults as necessary Provide a resource of advice and expertise for patients, their families and carers Communications Demonstrate excellent communication, verbal, written and IT skills To communicate with all levels of clinical and non-clinical staff in an effective manner Be sensitive to the needs of patients and carers who may have communication problems Provide a trust point of contact for accepting referrals Support the IV practitioner in delivering the core values and beliefs of the Trust Standards and quality initiatives Act as a professional role model, through commitment to the integration into practice of Trust Policies and procedures (i.e. Dress Code Policy) Communicate complex clinical information in an understandable form to patients/service users and carers in order to gain their consent for treatment Responsibility for Patient Care Assist in promoting and implementing an effective and sustainable IV therapy team Ensure that patient documentation is accurate, legible and complete, noting changes in the patient condition to the MDT Provide an environment that encourages patient centered involvement where patients ask for advice and education Work in an innovative and flexible way to meet the needs of the patient and carer Promote and maintain best practice Be proactive in the planning of safe timely discharges and communicate information to enable on going care management of the patient Planning and Organising Ability to prioritise workload and be flexible in response to individual and service needs Work in collaboration with patients/service users, carers and all statutory and voluntary organisations to ensure appropriate, safe and well-coordinated care delivery Ability to work in a variety of settings and continue to deliver high quality care Responsibilities for Physical and / or Financial Resources Be aware of cost implications associated with the service and ensure effective use of all resources Actively participate in Trust initiatives which contribute to the efficient management of department resources Be responsible for the use and security of assets within the department. Responsibility for Policy and Service Development and Implementation Reports all incidents and near misses appropriately as identified in the organisational policy Promote and maintain best practice that ensure the health, safety and security of self, patients and colleagues in accordance within organisational guidelines Assist in the development of the Community IV therapy team Work flexibly, prioritising work load to meet patient needs. Plan and organise own work load for week/month/year and be able to adjust workload for complex referrals that may arise Demonstrate competent IT skills to manage any required data collection Responsibilities for Human Resources and Leadership Maintain confidentiality of staff and patients at all times Ensure effective communication both within the department and as a department representative at meetings Seek support and advice on matters outside your experience and authority Responsibilities for Teaching and Training Undertake training and development in line within organisational policy, including mandatory training Contribute to the provision of IV therapy training across the trust Participate in the training/mentorship of pre-registration students and new employees Responsibilities for data and information resources To abide by all relevant Trust and Departmental policies including information governance, confidentiality and data protection Research, Development and Audit Demonstrate awareness of the research process and resources available Participate in clinical audit as required Ensure up to date knowledge of current evidence based practice. Implement research findings in collaboration with the MDT where appropriate, to provide a high quality, effective service Collect and input data for CQUIN, audit and other KPIs as appropriate Physical Skills and Effort The post holder must be physically fit to carry out the required duties within the hospital and community setting Ability to perform advanced venepuncture and cannulation skills General Duties Be familiar with and follow health and safety policy and procedures. Be aware of individual responsibilities under legislation, drawing any areas of potential risk to the attention of managers To assist the Trust in reducing healthcare associated infections. Be familiar with the Trusts Hand Decontamination Policy, attend mandatory training in Infection Prevention and Control and be compliant with all hand hygiene standards at all times All employees have a duty and responsibility to protect and safeguard children and vulnerable adults To undertake any other duty which is deemed appropriate to the band when requested by senior staff
