Summary

Job Description:

Lancashire County Council is seeking a highly experienced and knowledgeable Team Manager to join the Adult Services and Health & Wellbeing department. This role involves overseeing the intermediate care setting across Lancashire, with a strong focus on hospital discharge processes.

Key Responsibilities:

Leadership: Manage and lead a team responsible for intermediate care services, ensuring high-quality support and care for adults across Lancashire.
Hospital Discharge: Oversee and coordinate hospital discharge processes to ensure smooth transitions for patients into intermediate care.
Intermediate Care: Provide expert knowledge and guidance on intermediate care practices and procedures.
LAS Expertise: Utilize your extensive knowledge of the Liquidlogic Adults' Social Care System (LAS) to manage casework and streamline processes.
Collaboration: Work closely with healthcare professionals, social workers, and other stakeholders to ensure comprehensive care and support for service users.
Office Presence: Maintain a strong office presence at County Hall, Preston, to facilitate effective team management and collaboration.
Qualifications and Skills:

Proven experience in a similar managerial role within adult services and health & wellbeing.
In-depth knowledge of hospital discharge processes and intermediate care.
Extensive experience with the Liquidlogic Adults' Social Care System (LAS).
Strong leadership and team management skills.
Excellent communication and interpersonal abilities.
Ability to work effectively in a fast-paced and dynamic environment.
