Summary
Adults Social Worker
Warrington Borough Council are currently seeking a dedicated and compassionate Adults Social Worker to join their Community Discharge to Assess Team (D2A).
Responsibilities
- Conduct Care Act assessments of individuals who have been discharged from the hospital and require community support.
- Assess and evaluate the individual's physical, mental, and emotional needs, as well as their functional abilities, to determine the appropriate level of care and support required.
- Collaborate with individuals, their families, and other healthcare professionals to gather relevant information and develop comprehensive care plans.
- Provide advice, guidance, and emotional support to individuals and their families, helping them to understand their options and make informed decisions.
- Liaise with relevant community resources, such as healthcare providers and voluntary organisations, to ensure the seamless provision of services.
- Advocate for individual's rights and entitlements, ensuring that they receive appropriate and timely support services.
- Maintain accurate and up-to-date records of assessments, interventions, and outcomes in accordance with organisational and legal requirements.
- Stay current with changes in legislation, policies, and best practices related to social work, care assessments, and community support services.
Requirements
- Bachelor's or Master's degree in Social Work
- Valid social work registration
- Enhanced DBS
- Valid driver's license and own car with business insurance
- Proven experience in conducting assessments and developing care plans
- Experience in Discharge to Assess is essential
