Summary
To work in collaboration with the Ocean View multi-disciplinary team, to develop robust discharge plans with and for service users. This will involve working with service users from admission to the ward, through their in-patient stay, and post-discharge in the community. The post holder will apply the core principles of recovery in discharge planning and preparation, supporting the multi-disciplinary team to help service users to develop and practice essential skills for independent living. The post holder will support the initial assessment and discharge action planning for service users admitted to the ward, and provide follow through support once they have been discharged. The postholder will work closely with the social care assessment team on the social care pathway. Please see separate document for full job description.
