Summary
Clinical To undertake delegated duties to patients in their own home, residential care setting or clinic setting, this can include routine wound care, venepuncture, assisting in the delivery of palliative care, administration of insulin and eye drops following training and assessment of competencies. Communicates with the District Nursing Team members, other health care professionals and voluntary agencies discussing patient care as directed by the qualified nurse. Attends training relevant to role and ensures development opportunities are identified through the appraisal process. Undertakes clerical work within the team as delegated by the Team Leader such as stock ordering, filing and retrieval of patients notes. Responsible for maintaining a nursing bag with appropriate equipment required to deliver care e.g dressings and gloves. To participate in health promotion programmes with patients and carers, encouraging self-awareness and self-management. To assist in the support and education of family/carers to prevent ill health.Completion of the Care Certificate will be required. Communications Maintain confidentiality of information. Communicate effectively with staff at all levels including team members, patients, relative and other visitors to the Trust, acting at all times within the Trust Values and Behaviours. Possess the ability and knowledge to utilise different methods of communication with individuals and their families to provide support. Able to identify and develop methods to manage barriers to effective communication. Demonstrate excellent verbal and non-verbal communication skills to communicate sensitive information effectively with patients, families, colleagues and other agencies. Communicate appropriate information to and from other departments/service as required. Maintain accurate and comprehensive patient records in line with organisation standards of practice and data protection legislation. Demonstrate empathy with patients, their families and carers, ensuring their views, choices and experiences influence thinking and individualised care provision. Responsibilities for Physical and/or Financial Resources Contribute to the budget management by ensuring that the most effective use of resources is maintained and reporting any anomalies to the Team Leader. Promote a safe environment for the treatment of patients and protection of staff by ensuring the effective use of equipment and materials in compliance with Health and Safety Policies. Participate in product evaluation programmes if requested by the Matron.Ensure the regular maintenance of technical equipment including calibrating where required and checking for faults. Assist in the organisation of efficient stock control, maintaining adequate levels of stock and promoting the most cost effective usage of these resources Responsibility for Policy, Service Development and Implementation Be aware of, comment on and actively participate in changes in policies and service development Embrace and support line management and the team in making service improvements effective in the work place. To keep up to date with Intermediate Tier Service changes and identify opportunities for improvement and development of the services within the District Nursing Service. Attend and participate in training and external courses if agreed by Line Management. To implement changes in practice and service delivery as directed by the Matron. Responsibilities for Human Resources and Leadership Participates in the induction and orientation of newly appointed staff. Ensures compliance with Trust Mandatory Training. Works unsupervised the majority of the time, uses knowledge to care for specific cohort of patients. Monitor the health, safety and security of themselves and other members of the team, including identifying and assessing potential risk factors to maximise staff, patient and carer safety. Continuously upholds the core values and beliefs of the Trust. Acts as a professional role model. Contributes to the Clinical Governance Processes within the service ie prompt incident reporting. Demonstrate an overall awareness of quality issues and a commitment to continuous quality improvement. Research, Development and Audit Contribute to obtaining Friends and Family Feedback for the Service Identify areas where activity can be improved with support of the Team Leader Participate in audit as necessary Physical Skills and Effort The post holder will be expected to travel between different work locations throughout the day. They will be expected to load, transport and unload necessary equipment between work locations. The post holder will be expected to kneel, sit on heels, and move patients during manual handling, support patients while walking, assist patients to transfer, sit and stand. Mental EffortThe post holder will be expected to concentrate during care delivery and ensure good communication with patients who have impaired understanding and communication difficulties e.g. impaired speech and hearing. Emotional Effort Occasional exposure to distressing situations. The clinical work may involve working with terminally ill patients, having to provide support and advice to families/carers and patients in distress. The post holder will be dealing with people in their own homes who may be in the exacerbation stage of a long-term illness or a sub-acute stage of a new illness. Some clients may have poor family/social support. Patients may be depressed, withdrawn or show challenging behaviour.
