Summary

Role Post Title: Advanced nurse practitioner / paramedic practitioner Location: Fairfield Medical Centre Organisational arrangements (i) Reporting/Accountable to: Clinically - GP partners Administratively practice manager (ii) Responsible for: Scheduled & unscheduled patient care 1. Main purpose of the role To provide the local community with access to both scheduled and unscheduled care provision. The aim of the role is to : - deliver convenient accessible care - provide assessment, differential diagnosis and treatments of acute episodes of minor health and minor injury conditions - work in partnership with patients in agreeing management plans - promote self care in order that patients are empowered to manage their own health needs 2. Main responsibilities of the role - work as an autonomous practitioner, providing completed episodes of care to individuals - work within agreed guidelines and protocols where they exist and make autonomous decisions in the absence of guidelines - prescribe within knowledge and skills and in accordance with National and Local Guidelines - work within scope of practice and draw on relevant experience to deliver quality patient care - work in partnership with other health and social care providers - provide patients with the opportunity to contribute to the ongoing development of the service 3. Key areas accountable for 3.1 Clinical To conduct patient consultations using specialist knowledge in relation to minor injuries and health problems. Full assessments, history and differential diagnosis to be made in order that a management plan can be agreed with the patient. Interventions are timely, and may include provision of urgent care in an emergency. To contribute to the ongoing development of clinical guidelines reflecting best evidence based practice. To ensure that referrals to other primary and secondary services are made in line with agreed guidelines. To utilise diagnostic and clinical skills, developing new skills where appropriate. To prescribe according to and within scope of practice. Supply and administer medications in accordance with legislation and practice policies. To utilise clinical judgement, providing care and treatment within agreed guidelines where the need arises and within scope of practice. To have a working knowledge of child protection, the abuse of older people and domestic violence and refer to services as necessary. 3.2 Team work Ensure you are up to date with decisions made by the practice that might affect the care and support you give to patients. Share all major issues and new developments with the clinical team so that the practice can develop policies to make sure that all patients received the same standard and quality of care. 3.3 Communications Ensure effective communication with patients offering informed choice. Recognise and address patients anxieties and behaviours when managing acute health problems. Maintain effective communication with other team members ensuring all team members are kept involved and informed in the delivery and continued development of the practice. Patients are referred to primary and secondary providers with clear contemporaneous notes which accurately reflect the consultation and reasons for referral. Caldicott Guardianship is effective. Consent and Confidentiality is maintained in line with data protection and consent policies. 3.4 Clinical governance Quality controls and risk assessments are performed and forwarded for action. Awareness of health and safety for self and others and attend mandatory training. Give due consideration to the daily exposure to bodily fluids. All health and safety incidents are managed accordingly through the appropriate reporting mechanism. Medicines are managed safely in accordance with practice policy. 3.5 Education training Demonstrate awareness of own professional development and training needs in accordance with service needs. Promote and support the development and provision of work based learning to other clinical staff members. Act as a mentor for other clinical staff members. Continued professional development through reflective practice is integral to practice, clinical supervision is regularly undertaken. 3.6 Service development Ensure service is efficient by monitoring and exploring opportunities for savings. Undertake clinical audit to review practice and service, reporting results of audits. Explore opportunities in line with National Health plans to support effective health care for patients. 4.
