Summary

This role has a starting salary of £26,864 per annum, based on a 24 hours per week.


This is a 12-month fixed term contract/ secondment opportunity.

Contractual Location is open to discussion, can be Woodhatch in Reigate, Quadrant Court in Woking, or Dakota Building in Weybridge. However, this is a hybrid model of working remotely in the office and in community locations as required by the role.

About the Team
The Communities and Prevention Team sits within the Adults, Wellbeing and Health Partnership directorate. The Team of 15 people works across the wider determinants of health to promote prevention and early intervention, helping residents to remain health, well and independent for as long as possible.

The supportive team strives to be inclusive, develop accessible community support, collaborate and achieve high performance. We have a continuous learning and development approach, helping all team members to recognise their skills and achieve their best.

The team was shortlisted for a "Surrey Stars" awards in both 2022 and 2023.

About the Role
In Surrey, our ambition is to support a Thriving Communities approach, which enables residents to live independently and well, for as long as possible in their own communities. We want to embed a model of social care that grows local resources and encourages community behaviours which benefit the individual and enables the council to deliver services within the resources available. The Communities and Prevention Team Leader will support residents by ensuring advice and guidance on informal, free or low-cost community-based care and support is available at the right time and in the right way. As a result, you will help prevent, reduce, or delay care and support needs from developing, as required by the Care Act 2014.
We are looking for someone who is passionate about leading a team, developing processes and implementing change, working alongside social care teams and a range of professionals and volunteers. You'll work collaboratively across the social care system, including with Public Health, Surrey and Borders and Surrey Heartlands Integrated Care Systems and voluntary organisations.

The successful candidate will be required to work with social care teams across Surrey, with the expectation to travel around the County to meet the community and partners in various locations. In line with SCC policy, efforts will be made to reduce the travel required where possible. Flexible working will be supported through mobile technology. This role supports a hybrid model of working both from office bases across Surrey but also remote working.

Shortlisting Criteria
As part of your application, you will be asked to upload a CV and answer the following 4 questions:

  1. Describe a project you have managed that focused on community capacity building or community development. What were the key objectives, and how did you achieve them?
  2. Explain your understanding of the current pressures facing Adult Social Care Services. How have you applied this knowledge in your work to support prevention initiatives and empower communities?
  3. Provide an example of a time when you inspired and managed a team to ensure quality service delivery. What approaches did you use to motivate your team, maintain high standards, and achieve your goals?
  4. Discuss a situation where you successfully built relationships and worked within cross-organisational teams to deliver innovation and change. What obstacles did you face, and how did you overcome them?

To be considered for shortlisting for this position, your answers to the questions above and your CV will clearly evidence the following:

  • Experience of community capacity building or community development, including project management skills
  • A good understanding of current pressures on Adult Social Care Services, as well as have a good understanding of our ambitions around prevention and empowering communities.
  • Proven ability to inspire and manage a team, and ensuring quality service delivery
  • Proven ability to build relationships, network and work in cross-functional and cross-organisational teams
  • Evidence of persistence and the ability to overcome obstacles, to deliver innovation and change
