Summary

Contributes to resource management. Assist the acute and community dietitian and associated nursing staff with effective discharge planning from hospital facilitating the safe discharge of patients requiring home enteral tube feeding and training patients /carers in the use of equipment. Assist with the smooth running from referral of patient to service delivery and review. Provide a high standard of support to the community home enteral feeding dietetic service through monitoring of patient requirements using evidence-based practice. Organise dietetic and nursing resources to meet patient needs. Manage own time appropriately and work effectively as an individual and/or as part of a multidisciplinary team. Record all interventions in dietetic record cards, nursing notes and clinical notes in line with department and HCPC Standards. Ensure accessibility of Home Enteral Feeding/community resources and equipment. To monitor and facilitate the provision of prescribable nutritional products to community hospitals and patient homes, as requested by the dietitian. Liaise with catering services to ensure correct provision of therapeutic diets to patients at assigned care homes, under the guidance of the dietitian. To participate in the review, update and development of evidence-based department resources. Assist dietitians with presentation preparation / literature searches to aid evidence-based practice. Ensure confidentiality and information governance compliance. To work autonomously and adhere to the lone working policy. To proactively contribute to the achievement of clinical outcomes through individual and team effort. Assist dietitians with student training. Participate in audits and quality monitoring. Attend and contribute to department meetings
