Summary

Duties and Responsibilities
1. To be the single point of contact (SPOC) in the co-ordination and the
effective delivery of health and wellbeing priorities for residents residing
in the Links accommodation.

1.1 Provide co-ordinated health support that is safe, timely, effective, efficient,
equitable, and young person centered for individuals referred for a service.

1.2 Work flexibly to attend appropriate meetings and interventions and be
responsible for accurate record keeping.

1.3 Utilising a range of outcomes and impact assessment tools, identify resident
health needs, health and wellbeing awareness and resilience.

1.4 To attend and represent the Association across varied partnerships in respect
of health and wellbeing related principles, develop partnership opportunities
and share achievements and best practice.

1.5 To work to a range of agreed service specifications and service level
agreements as defined within the Association’s Business Plan.

1.6 To equip residents with the additional confidence needed to support their
journey towards independent living.

2. To support initiatives to actively promote healthy lifestyles and improve
social well-being for residents.

2.1 To provide information, advice and guidance to residents in relation to their
health and wellbeing options and opportunities.

2.2 To assist in the development of support plans and timetables to improve health
and wellbeing awareness and services to residents and improve their quality of
life.

2.3 To design, deliver and monitor health and wellbeing programmes, events and
activities for residents, linked to the Short-Term Outcomes framework.

2.4 To engage with other colleagues in the delivery of joint programmes and
activity.

3. To engage and empower residents to reduce the impact of risk factors
that result in poor health.

3.1 To encourage residents to prioritise their health and wellbeing needs through
workshops and one to one sessions.

3.2 To undertake appropriate assessments for the delivery of risk-taking activity
and ensure that residents who engage in unhealthy, unplanned and potentially
unacceptable behaviour are steered towards healthy and meaningful
alternatives.

3.3 To be knowledgeable of professional services and to signpost residents to
ensure they are getting the best advice and treatment.

3.4 To ensure that a high-quality outcome focused service is received from the
required health care and wellbeing services.

3.5 To act as an advocate for residents, when additional support is required, in the
following areas; physical health, smoking, drugs, alcohol, sexual health, mental
health, traumatic experiences and treatments.

4. To promote protective factors using models that prepare residents for
active citizenship.

4.1 To promote active citizenship through active engagement in health and
wellbeing initiatives, taking place in any of the Association’s projects or in the
community.

4.2 Promoting self-advocacy in order to increase resident confidence in attendance
at hospitals and general practice centres etc.

4.3 To guide residents in themes that help them to improve their quality of life
through confidence building, negotiation skills, interdependency and self-sign
posting.

4.4 To support residents to access services that develop their knowledge e.g.
general first aid.

5. To promote and contribute to all Association initiatives that creates
supportive environments.

5.1 To support and advise colleagues on initiatives that assist in the creating of
healthy environments for residents.

5.2 To support the promotion of, good health and hygiene in the home environment
and to encourage and promote healthy communal living arrangements.

5.3 To produce appropriate marketing and publicity materials for the events and
activities carried out.

5.4 To assist in securing additional funding for events and activities for residents.

5.5 To monitor the diversity profile of residents who engage in health and wellbeing
programmes.

5.6 To be fully aware of Safeguarding procedures and protocols to pass on to
residents engaged in programmes.

5.7 To undertake appropriate recordkeeping relating to activity and events, by
updating tasks and support planning arrangements, act activity data records in
accordance with the Association and Wirral’s Mainstay systems.

5.8 To develop and maintain appropriate recording and administrative systems to
demonstrate progress in meeting Association targets and outcomes.

6. Ofsted Standards: For Ofsted registered sites, to adhere to all Ofsted
regulations and standards, provide quality and consistency of service and
property standards to continually meet the Ofsted regulations, and be
part of the preparation for Ofsted inspections.

6.1 Be responsible for ensuring the site and services provided meet the
requirements of Ofsted.
