Summary
The post holder is responsible for the development, delivery and evaluation of clinical pharmacy services to Rheumatology Department at Croydon Health Services NHS Trust (CHS). The post holder will provide expert clinical pharmacy advice to all staff and patients associated within the Speciality of Rheumatology, which will be their main area of clinical practice.
They are expected to work towards or be able to prescribe and involved in prescribing of cDMARDs and bDMARDs within Rheumatology. They are expected to act as lead directorate pharmacist for Rheumatology and carry out directorate tasks such as completing formulary applications, writing local guidelines and policies, and presenting them at appropriate committees as well as line manage and involved in staff development.
The will be working closely with specialist Rheumatology nurses and consultants and expected to work collaboratively in developing and improving services provided within Rheumatology.
They will act as a role model in the Speciality of Rheumatology and undertake clinical supervision of pharmacy staff and teaching of nursing and medical staff

Main Duties and Responsibilities
General Duties:
• Leadership: Lead and evaluate clinical pharmacy services in Rheumatology.
• Specialist Service: Provide expert pharmaceutical services, focusing on biologic medicines and biosimilars.
• Collaboration: Work with nurses and consultants on therapy switching and biologics review clinics.
• Patient Care: Ensure safe, effective, and cost-efficient drug use.
Clinical Practice:
• Prescribing: Work as a non-medical Independent Prescriber.
• Support: Provide pharmaceutical advice and support safe prescribing.
• Documentation: Accurately document drug histories and communicate changes to GPs.
Homecare and High-Cost Drugs:
• Management: Oversee homecare services for Rheumatology medicines.
• Stakeholder Relations: Maintain relationships with homecare providers.
• Incident Review: Review and address adverse incidents related to homecare.
Management and Development:
• Staff Management: Train and develop Rheumatology and Pharmacy staff.
• Training: Educate medical, pharmacy, nursing, and other healthcare groups.
Financial and Clinical Risk:
• Advice: Provide high-level medicines management advice.
• Safety: Report and investigate adverse events and medication incidents.
• Governance: Conduct clinical audits and develop safe systems.
Governance:
• Initiatives: Implement clinical governance initiatives for medicines use.
• Risk Mitigation: Develop strategies to reduce risks in medicines use.
