Summary
Support Worker - Young People’s Long & Short-Term Accommodation

£13.00 per hour, Fixed 16 hours per week
About the role
To provide early and targeted support to aid the recovery and resilience of people with multiple vulnerabilities, working across a portfolio of longer-term accommodation-based services intended to positively address the ‘revolving door’ nature of shorter term supported accommodation. Demonstrating a commitment to safeguarding in line with policy and procedure you will support care leavers to develop the knowledge and skills they need to achieve their goals, empowering them to build successful relationships within their community and access the necessary services to develop skills to better manage their health and wellbeing and increase levels of independence as they move through a pathway model.
This post does not involve any personal care responsibilities.

About You
We are looking for individuals with an NVQ L3 in Health & Social Care and any relevant experience in providing specialist support that may and will cover a range of activities:
Excellent communication skills as well as a user focussed approach are essential
Proven ability to work effectively with a wide variety of stakeholders, both statutory and non-statutory.
Experience of agile working across locations to support a case load of customers, all provisions are within a 3 mile radius of each other. Driving licence essential.
A working knowledge of legislation in regard to delivering support.
Experience in facilitating involvement and engagement from service users.
IT Literate and competent with Word and Excel. You must be able to maintain accurate records that capture identified outcomes and Key Performance Indicators.
Ability to manage competing priorities and work to deadline.
Understand the wider strategic and legislative framework and associated outcomes.
An empathetic nature with the passion to support vulnerable young care leavers.
