An exciting opportunity has arisen for a Full time equivalent Care Co-ordinator to join the Assessment and Review Team.
Successful applicants will be supported by a Team Manager and will work in collaboration with colleagues from other disciplines within the team which will comprise Occupational Therapists, Social Workers, community care assistants and technology staff.
The team is responsible for the assessment for new community services, review of existing care at home services and oversight of unmet need in relation to Care at Home.
Assessments adopting a strength’s- based approach are undertaken in a multi-disciplinary context with a focus on collaboration between all members of the team to secure the best possible outcomes for the people being assessed
Successful applicants must have the ability to work as part of a team, respond to the demands of a busy service, be able to follow procedures, have excellent communication and good report writing skills. They must have the ability to travel throughout West Lothian and hold a recognised qualification in social care.
The skills and knowledge applicants must have are: -
SVQ 3 and an HNC in Social Care or equivalent.
It is essential for candidates to have relevant assessment and care management experience in an adults or older peoples social work practice team.
This post is considered Regulated Work with Vulnerable Children and/or Protected Adults, under the Protection of Vulnerable Groups (Scotland) Act 2007. Preferred candidates will be required to join the PVG Scheme or undergo a PVG Scheme update check prior to a formal offer of employment being made by West Lothian Council.
