Summary


Essex Learning Disability Partnership provides specialist learning disability services across all Essex. The partnership has been formed by Hertfordshire Partnership NHS Foundation Trust providing Learning Disability Services in Mid, North East and West Essex and; Essex Partnership University NHS Foundation Trust providing Learning Disability Services in South Essex.

The Partnership was formed in 2018 and has been developing new pathways of working that seek to improve health outcomes for people with LD making sure that they are able to access the right care in the right place at the right time, in accordance with Building the Right Support. The successful candidate will have the opportunity to be part of this continuing journey and influence how we work to improve lives.

Team members have won awards for innovations in practice and mentoring students. The Clinical Lead Nurse has been awarded the title of Queen's Nurse and the STOMP clinic set up by two non-medical prescribers has been recognised by NHSE as an example of best practice. The Significant Risk Tool developed by two members to identify frailty in people with learning disabilities has been nominated for an award and is also recognised by NHSE as an example of best practice.

The successful candidate will also have the opportunity for continued personal and professional development with opportunities for apprenticeships.

The role of the Community Support Worker – Enhanced Support Team is to assist the nursing team in providing holistic and effective nursing care to those people with learning disability requiring intensive Support during time of mental health or behaviour crisis. The post holder will support the wider MDT to support people with learning disability and their families and carers during a period of crisis to try and reduce the need for admission to hospital. Where hospital is required to support a timely discharge back into the community. In addition ensuring that people with a learning disability have equitable access to mainstream mental health services and that reasonable adjustments are made to promote recovery.

The Mid and South Essex Integrated Care System (ICS) brings together key health, care, community, and voluntary sector organisations from across our area. Our aim is to work together in partnership to join up health and care services, improving our population’s health and wellbeing and reducing health inequalities. One such example of this collaborative work is the launch of our HCA Academy, an ICS initiative between Essex Partnership University NHS Foundation Trust (EPUT) and Mid and South Essex NHS Foundation Trust (MSEFT) to support the incoming Health Care assistant (HCA) workforce. The academy models a ‘one workforce’ cross system approach to the onboarding, induction, education and experience of Healthcare Assistants.

To support this ‘one workforce model’ the successful candidate for this post will be required to attend our 5 day (Monday to Friday) face to face Induction Programme at our MSE ICS Education Hub at Brentwood Community Hospital. Your start date will therefore fall on one of two of the HCA Academy induction dates across the month, which will be discussed and agreed during the recruitment process.

Our HCA Academy Induction programme has been designed to train and support the successful candidate through the early stages of their employment at EPUT and will provide opportunities to enhance skills, knowledge and experience.

This will be followed by an additional 5 day (Monday to Friday) EPUT corporate / mandatory training welcome. (EPUT Welcome will be varied between face to face and virtual delivery).
