Summary

The Post Holder:Is a member of the South Hams and West Devon RRCS and is attached to a Community IC team working over a 7 day pattern. Has responsibility for taking Rapid Response referrals and activating these services within the specified response time. Will take referral information in a consistent manner, gathering additional information, arranging assessment, initiate and co-ordinate care with feedback to referrers. Will review and co-ordinate needs of service users with professionals. Will programme staff resources to meet the specified needs of the service users. Will Deal with less complex referrals and forward to appropriate area, e.g. Social Services Will order Stock co-ordinate ordering of supplies via the procurement system to the Business Manager(budget holder) requirements ensuring these are authorised by the budget holder and receipted as items received. Will link into as required to access patient details in relation to Rapid Response. Will produce statistical reports as required by the Rapid Response Team Leader. Will Work with a wide range of information- handling requirements on behalf of two or more agencies e.g. Torbay and Southern Devon Healthcare Trust (TSDH CT), Adult & Community Services (ACS), Devon Doctors on call services. Must have a high level of typing skills and IT Skills involving extensive experience of Microsoft Office including Word, Excel and Outlook. Will actively maintain positive working relationships with colleagues and within the multi-disciplinary team Will pay particular regard to the Health and Safety Issue when Lone Working in line with NHS Trust Policies and procedures. Will meet targets/objectives as agreed by the Team Leaders and Service Manager. Has responsibility for monitoring emails and actioning where necessary. Will maintain accurate records both electronic and written. Has the ability to work flexibly and adapt to changes. Ability to use own judgement and initiative. Will liaise with other health, social or voluntary care services and agencies to ensure identified patient care needs outside the scope of the team are met. Will annually identify written personal objectives with the RRCS supervisor / Manager. May need to change base, within the localities, to meet the needs of the service or Livewell SouthWest. Will need to be able to drive and have access to a roadworthy vehicle during as the role is based within a rural setting. DIMENSIONS Exercising greater levels of own judgement and decision making based on an assessment of risk and within agreed limits. Providing a consistent approach with incoming calls in line with service provision and prioritising workload to ensure deadlines met. Take responsibility for sensitively handling enquiries that might arise when Line manager absent referring on to appropriate person when necessary. Dealing sensitively with the users of the service and other members of the public. Responsibility to deal confidentially and sensitively with a variety of patient information System 1 or equivalent (ACS) Obtain patient/client information as required for Intermediate care and input into system as required. Working with two IT Systems Health and Devon County Council (ACS) Supporting Joint Agency Staff and managers when necessary working in an operational way. Demonstrate evidence of own competencies and continuous personal development through reflection, supervision and staff development and review sessions. Primary duties and responsibilities The post holder is responsible for the Referral and Coordination of Rapid Response Care service in the South Hams and West Devon localities. The post holder needs to be able to co-ordinate detailed referrals from Health and Social Care Professionals, communicate and answer queries from all disciplines including Care Providers, members of the public and users of the service. This involves organising own day to day activities, planning of both straightforward and more complex on-going referrals, together with looking into the longer term planning of care. This is a busy and often pressured role; post holder will need to remain calm under pressure and work to a high degree of accuracy with conflicting demands. PRIMARY DUTIES AND AREAS OF RESPONSIBILITY To be able to co-ordinate detailed Rapid Response Referrals from Health and Social Care professionals, communicate and answer queries from all disciplines. To organise own day to day activities, planning of both straightforward and more complex on-going referrals, together with looking in to the longer term planning of care. To have high speed, accurate, typing skills Quick response times are required in order to provide a fast effective programme of care for patients. Effective communication is required to ensure patients receive the correct information and care, specific to their needs. To Record statistical information, recording own information, entering data, using computer software, transcribing minutes etc. This involves inputting, storing & providing information, modifying, maintaining, and analysing information, together with working within team and Line Manager to bring about system changes. To Make links with local community resources and networks To Provide information and advice on how to access services that will benefit the health and wellbeing of individuals. To contribute towards ensuring a safe working environment for self and others and report on situation of potential risk To be conversant and compliant with LSW policies and procedures relating to clinical governance and that their requirements, along with nationally recognised professional standards, are incorporated within clinical practice. Ensure that LSWs policies and procedures relating to health and safety at work are understood and implemented. Clinical Direct and Incidental contact is required with both patients and internal/external colleagues. To Provide schedules of care as required by the referrer. To support and encourage patients, their relatives and carers in an active approach to regaining and maintaining maximal functional independence. To work closely with other RRCS colleagues and IC team to ensure patients identified goals are met. To be aware of, and act upon, LSW health and safety policies, including the prompt recording of any accident, incident, or other untoward event involving self, patients or other staff. To undertake any other duties considered reasonable and appropriate by RRCS manager. To contribute to RRCS service development. To contribute in audit and clinical governance activities. Management and Leadership 1.The postholder should work both individually and within the Team to contribute to service improvement. The Postholder to undergo monthly supervision and annual appraisals The postholder has responsibility for ordering stock via the Trust procurement process responsible to Line Manager. The postholder has responsibility for working without supervision, and should adhere to the Lone Working Policy. To be responsible for using own initiative and be flexible when organising support workers caseload on a day to day basis, which is often unpredictablein order to respond to the needs of the service. To plan visits in such a way as to make best use of resources such as time, skills and expertise, as well as mileage. This requires a high level of time management, networking and organisational skills. To undertake clerical duties this may include ordering of stores and stationery, storage of patient records, photocopying, shredding of paper, maintenance and monitoring of equipment. Monitoring and ordering of stock levels and to utilise resources economically and effectively and promote cost consciousness throughout the service. To participate in team activities so as to develop and consolidate a cohesive and supportive team ensuring openness within the team. Education 1.To maintain your competencies through mandatory training and external training opportunities where appropriate. To participate in the LSW staff Appraisal Scheme including the formulation of Personal Development Plans. To actively participate in personal development training needs. Communications and working relationships 1.To Communicate with other team members of Multi Disciplinary Team, Health & Social Care Professionals, Members of the public. This involves a complete range of personnel on different levels of communication. This involves both confidential and sensitive complex communication. Ensure that patients, relatives, carers and professionals are kept fully informed of progress following intervention. Due to the diverse and complex nature of the caseload, this requires the ability to communicate complex information to meet a range of levels in understanding, and to overcome such barriers as, eg. hearing loss, cognitive impairment, dysphasia, pain, fear, social and cultural differences. Able to communicate the role and eligibility criteria of the RRCS Service. Act as a patient advocate as required ensuring individual needs, preferences and choices are delivered by the Service. The Post Holder to be supplied with mobile telephones and lone working devices for work purposes and be expected to abide by LSW policy. To maintain and Preserve confidentiality at all times, and be aware of the Data Protection Act, Access to Health Records and Consent for Treatment. To collect and enter statistical data relevant to the service in line with LSW and RRCS policies and procedures, this includes audits.
