Summary


We are looking for a dynamic and motivated Physical Activity Specialist with a passion and specialist interest in cardiac rehabilitation.

The cardiac rehabilitation service offers phases 1 to 3 to patients following heart surgery, PPCI, MI, angioplasty procedures and heart failure etc.
You will be assessing, teaching and supporting patients in their recovery on the telephone, face to face, and virtually as part of the hospital based cardiac rehabilitation programmes. As part of a multidisciplinary team you
will work with patients encouraging and supporting them to return to optimal health.

The role includes patient assessment, delivering individual and group education, supporting patients and delivering group and 1:1.

This post will involve cross site working.
