Summary


Hi, my name is Louise Lewis. I am a community matron working for Manchester Community Response services and we are looking for an experienced and motivated Band 6 Nurse to join our supportive, dynamic Manchester Community Response (MCR) Hospital at Home team at North Manchester.

The post will be based in B4 at North Manchester General Hospital which is part of our Manchester Community Response Service which also includes the Crisis Response team, Home Pathway, Reablement, Community IV team, Bed based Intermediate care, Pharmacy and Social care.

This is an exciting time to be part of our integrated multi-disciplinary and multi-agency service enabling patients to receive treatment in their own home whilst remaining under the care of a consultant.

The post holder will work as part of the Hospital at Home Team to co-ordinate the admission and discharge of selected patients and be involved in the Rapid Response function of the service. The post holder will be working as part of a multidisciplinary and multiagency team and in collaboration with other services including hospital staff and community teams to facilitate the timely referral, assessment and appropriate discharge from hospital through assessment, care planning and integrated working responsive to patient needs.



The post holder will provide and/or arrange appropriate support and interventions in the community working closely with colleagues across all sectors. The post holder will provide specialist knowledge and clinical skills to ensure the delivery of high-quality safe care within the service and in the wider community and across the acute/community interface. He/she will actively participate in improving the service and will initiate and support future developments.
