Summary
The Special School Nursing Service (CYP Team Lead post is a new leadership role based within the Children and Young People’s Community Nursing Service (CYPCN) in Sussex Community NHS Foundation Trust (SCFT). Leadership is focused on the clinical, public health and learning disability nursing of children and young people (CYP) in special school settings. This involves working closely with families and school staff, as well as undertaking operational performance review of the service.
The Team Lead will be responsible for leading the Special School Nursing service operating in identified Special Educational Needs & Disability (SEND) schools in the West Sussex and Brighton and Hove area. They will ensure a standardised, equitable service is delivered across the locality, acknowledging differing caseload complexities. They will contribute to the development of the Sussex countywide CYPCN Service together with other senior nurses, ensuring policy and practice is developed and delivered in line with Trust and national best practice standards.
The Team Lead will have an approximate 75% operational: 25% clinical delivery role; a key element of this role is in performance management for the delivery of the SSN service, including the responsibility for day-to-day line management of the SSN team, including authorising leave and travel expenses. This role will include providing clinical delivery of care in a West Sussex and Brighton and Hove Special School.
· Work collaboratively with the Clinical Lead and Practice Development Nurses to support and direct the team
· Provide day-to-day operational leadership for the team.
· Manage a small caseload of CYP attending Special School’s in an identified locality.
· Lead and manage the delivery of quality nursing care, support, information, and advice and ensuring that appropriate resources are available to provide care for CYP whom access the identified SEND schools in West Sussex and Brighton and Hove.
· Support and educate staff offering direction and leadership surrounding delivery of nursing care for the SSN caseload and continual professional development.
· Maintain excellent communication channels with service stakeholders. Engaging in flexible, cross boundary partnership working including the CYP and family/carer, to develop seamless service provision and deliver safe quality care.
· Staff may be required to work anywhere in West Sussex and Brighton and Hove to meet the needs of the commissioned service.

The Team Lead will ensure a safe and consistent nursing service is delivered to CYP on the register of the identified SEND Schools. This includes assisting with prioritisation and ensuring equity of service delivery across the caseloads.
The Team Lead will set and maintain clinical and service performance standards in the SEND schools as well as supporting the wider CYP Community Nursing
Service with care standards as necessary.
The Team Lead willlead and manage the day-to-day operational nursing, personnel, performance, and allocation of work, reporting directly to either the Matrons or General Manager.
The Team Lead will support the Clinical Leadondeveloping a countywidehealth needs profile and work with the Matrons and General Manager to secure funding to establish provision based on identified need.
The Team Lead willlead on staff development and performance within the team and ensure that all appraisals are completed annually and ensure feedback onperformance is given at regular intervals throughout the year in conjunction with Clinical Lead.
