Conway Respite Services

The post holders for the support worker role will be experienced, motivated, flexible, and enthusiastic care staff to join our team, in a short break setting to provide care and support to individuals with a learning disability, you will be required to follow individual care plans, and risk assessments.

You will need to follow Person Centred Plans that are detailed to meet individual needs, implemented, monitored, and reviewed with all staff and carers involved.

Contribute towards the monitoring and assessment of service users plans, reviews and assessment of risks ensuring compliance with organisational policies and procedures in line with Care Quality Commission Essential Standards of Quality and Safety.

Work with the team to provide sensitive care and guidance to service users in areas of personal care, daily living, health, social integration, and emotional support.

Good communication skills are essential, and you will need to have the ability to use your own initiative and work unsupervised while out and about with service users. You will have the ability to write in daily notes within the service setting, and use IT equipment.

You will be required to work on a rota system, days, afternoons & nights over a 7-day week which includes, weekends and bank holidays.
